HIV and Quality Improvement Nurse (RN)

Project Renewal is one of the larger social service nonprofits in New York City with Federal Qualified Health Center/Article 28 Diagnostic and Treatment Centers, OASAS licensed sites in addition to mobile health unit. Empathic, trauma and harm reduction informed services are delivered at low or no cost without barriers to our clients who often have severe mental illnesses, co-occurring substance use disorders, and multiple chronic medical conditions.

Title:                  HIV and Quality Improvement Nurse (RN)

Program:           Healthcare Department 

PRI seeks a registered nurse for a new hybrid clinical and administrative role. Clinically the RN will support patients and clinic staff with HIV care management, prevention, testing, and counseling. Administratively, the role leads and collaborates on quality improvement efforts to the HRSA funded HIV program activities, data analysis, reporting, and chart review reconciling external documents for patient records. Reports to: Associate Director of Primary Care Nursing

Essential Duties and Responsibilities:

The essential duties of the HIV and Quality Improvement Nurse include but are not limited to the following activities:

Clinical

• Participates in weekly HIV clinical meetings. Provides initial health assessment for each client. Helps to identify medical problems with clients.
• Works with clinical team to insure coordinated patient care.
• Provides counseling along the HIV care continuum including testing, Pre-exposure prophylaxis (or PrEP), and Post-exposure prophylaxis (PEP).
• Performs HIV testing for individuals and counsels on risk.

·      Provides routine monitoring of clients’ physical condition, triage of medical and psychiatric problems.

• In collaboration with HIV Specialists, Medical Director, Clinical Case Manager, and other providers, evaluate patient data and establish patient treatment plans.
• Work with HIV Specialists and Clinical Case Manager to monitor patients’ response to HIV medication.

·      Works with patients at risk to prevent new HIV infections by providing PrEP medication adherence counseling.

• Develop and lead support groups on PrEP medication adherence.

·      Supports patients living with HIV with medication adherence and counseling to achieve and maintain viral suppression.

• Collaborate with facility behavioral health programs on patient needs for HIV prevention and management.

·      Facilitates referrals to community-based psychiatric, and other community-based resources to address social determinants of health.

Quality Improvement(QI) and Programmatic

• Lead a new program initiative on HIV prevention promoting the use of PrEP and HIV testing at all PRI clinics.
• Participate in HIV related educational events with patients including quarterly Client Advisory Board.
• In partnership with Clinical Case Manager, provides outreach to clients when necessary, and in accordance with program policies.
• Participate in state and federal Ryan White Program HIV trainings, conferences, and meetings.
• Maintains documentation in compliance with agency policies and procedures.
• Reconcile external documentation for patient records as needed to maintain ongoing quality improvement efforts.
• Track HIV client progress to support federal and state reporting requirements.
• Monitor performance data and metrics on PrEP prescriptions and medication. Perform chart reviews for PrEP.
• Support CQI Project Manager, with HIV related data for internal and external reporting requirements.

Note: This role will travel as needed to support HIV services and trainings at all PRI clinic sites.

Qualifications:

• New York State Licensed Registered Nurse
• Working with people experiencing homelessness preferred.
• Must have excellent oral, writing and listening skills. 
• The ability to work independently, prioritize tasks, and create and coordinate schedules 
• Must have the ability to interface with clients as well as all levels of staff.
• Must have knowledge of Microsoft Office, especially Word and Excel.
• Knowledge of eClinicalWorks is a plus.
