Community Access, Inc. (CA), founded in 1974, is a progressive not-for-profit organization that expands opportunities for people living with mental health concerns to recover from trauma and discrimination through affordable housing, training, advocacy and healing-focused services. We are built upon the simple truth that people are experts in their own lives. Our programs are spread across 20 locations and include over 1000 units of supportive housing in three boroughs, the Howie The Harp Advocacy Center, ThriveAtWork, Blueprint Supported Education, East Village Access—a PROS program, and a peer-driven Crisis Respite Center.

HARM REDUCTION COORDINATOR
This is a full-time position with excellent benefits package.
The Harm Reduction Coordinator uses the model of harm reduction to assess tenant’s needs for services, including outreach, counseling, education, referral, etc., at program sites and plan services accordingly; Develops and supports strategies to meet the needs of specified groups; Provides training and education on the harm reduction model; Provides service coordination to assigned tenants; Provide recovery oriented support services and outreach to assigned program participants,; Develops and assists in implementing service plans determined by goals of program participants, including reviewing service plans with program participants, identifying progress and revising plan as needed; Produces and maintains thorough, accurate and timely documentation; Assists program participants by advocating for quality care from external service providers; Assists program participants in maintaining apartment free from unsanitary conditions and safety hazards, utilizing individualized strategies; Maintain a resources list and provide referrals for treatment; Assist program participants in identifying vocational and educational goals and opportunities; Assist program participants with conflict resolution; Provide outreach to program participants who are not indicating their goals or who are not engaged, to offer services to them, and to incorporate outreach in their service plans; Provide crisis intervention, as necessary.
Qualified candidates must have an understanding, appreciation, and commitment to the philosophy and mission of Community Access, with particular expertise in harm reduction and recovery oriented services; Minimum of a high school diploma or equivalent (GED) – Bachelor’s degree, preferred; 3 - 5 years’ experience working with substance abuse and psychiatric disabilities; Thorough understanding of harm reduction; Commitment to recovery-oriented practice; Must be fingerprinted and cleared by the NYS Justice Center; Skilled in articulating program goals; Be skilled in conflict mediation/negotiation and have an assertive approach to problem solving; Demonstrated leadership skills and ability to work as part of a team; Excellent oral and written communication skills; Ability to utilize various computer; rograms, specifically Microsoft Word and Excel; Be creative and flexible; Show initiative and be responsible for follow through; Ability to maintain confidential information, as related to position; Bilingual Spanish-speaking, preferred. Ability to speak other relevant languages, dependent upon specific needs of tenants.
