The Patient Care Coordinator will work with the Chief Medical Officer, Medical and Patient Support Services Units, Physicians, Patient Care Coordinators, other licensed providers and members of the medical services team to perform medical assistant responsibilities and to ensure patient adherence to recommended health maintenance and treatment guidelines as individuals and program-wide to enhance patient adherence and ongoing improvements of clinical indicators. Quality of care and patient satisfaction is the main goal.

Duties & Responsibilities:



1.    Schedule patient appointments, including new patient orientations.

2.    Review EHR before clinic to flag pending health maintenance and treatment issues.

3.    Monitor outcomes for individual patients and the aggregate.

4.    Conduct intake and exit interviews of clients during sessions and enter encounters into data system.

5.    Arrange and ensure follow-up internal and external referral appointments and specialty care for patients.

6.    Monitor patient panel for preventive measures, urgency, and other pending follow-up.

7.    Provide patient education around treatment adherence; STD and HIV prevention; healthy habits, and other issues as appropriate.

8.    Develop treatment adherence plans with clients and physicians, and monitor progress and outcomes.

9.    Support clinical functions with any chart reviews necessary for reports

10. Respond to patient administrative needs during the week.

11. Document all follow-up activities and phone conversations related to the patient in the patient’s medical record.

12. Participate regularly in huddles, triads, clinical team meetings, interdisciplinary case review meetings, and monthly staff meetings

13. Follow all La Clinica policies and procedures.

14. Perform other duties as assigned.


1.    Coordinate clinic sessions and manage clinic flow.

2.    Carry out clinical procedures including aseptic procedures, vital signs, set up clients for clinician’s care, venipunctures and nonintravenous injections and observe and report client’s signs and symptoms.

3.    Administer first aid.

4.    Assist with client examinations or treatments.

5.    Operate medical equipment in the office.

6.    Administer medication as directed by licensed clinician.

7.    Perform on-site laboratory services and patient procedures (including simple dressing changes, urine dipsticks, finger/heel sticks, A1C, EKG’s; adult/pediatric immunizations, injections, and venipuncture) ordered by a physician.

8.    Respond to patient medical needs during the week.

9.    Orient new PCC employees.

10. Other duties as directed by licensed provider.


·      High School Diploma

·      Registered or Certified Medical Assistant

·      Six months to one year clinical, plus hands-on medical assistant experience

·      Prefer one year experience with HIV positive clients

·      Two years prior experience working in a Latino community/populations, preferred.

·      Health education experience, preferred

·      Phlebotomy experience strongly desired

Level of Language Proficiency

·      Bilingual, Spanish-English.
