Job Type: Part-Time – 3 days/week – 21 hours

The Heights and Hills Caregiver Program is an innovative, robust program designed to support caregivers of adults over the age of 60. Currently staffed by four LMSWs, the program serves over 1300 caregivers annually. The program provides direct services to caregivers, offering supportive counseling, support groups, information and referrals, and assistance with benefits and entitlements The program also conducts extensive outreach in the community, offers a variety of workshops and trainings at settings throughout our catchment area for community and corporate partners, and provides limited respite services. The program has grown to serve 11 Community Districts in Brooklyn and is integrated with Heights and Hills’ other programs to provide a continuum of services to older adults and their families to achieve our mission of promoting successful aging in the Brooklyn community. 

Alzheimer’s disease and other related disorders have become the 5th leading cause of death in people over the age of 65 in our country, and the cost of caring for those with Alzheimer’s is estimated nationally at over $200 billion. The greatest cost, both financial and emotional, is borne by the families who provide care for those who have the disease. Unfortunately, the vast majority of caregivers are unaware of the availability and value of the types of supportive services the Caregiver Program provides. This is more so for the African/Caribbean communities of Brooklyn, a traditionally underserved community. According to the Alzheimer’s Association, studies show that “older African-Americans are about twice as likely to have Alzheimer’s and other dementias as older whites”. It is these statistics that have led Heights and Hills to create this new Caregiver Outreach Worker position to specifically target faith-based organizations and churches in Brooklyn, traditionally spaces that are safer and more welcoming to these communities.

Role

The Outreach Worker will conduct outreach to houses of worship and faith-based institutions within our catchment area with a focus on those serving the Afro-Caribbean community. The outreach worker will be responsible for making connections and having a presence in the community, and ultimately conduct trainings and workshops in order to make caregivers in under-served communities aware of the services available to them. The Caregiver Outreach Worker works under the supervision of the Caregiver Program Director and will work closely with program staff and other professional team members to cultivate relationships with community partners. The Caregiver Outreach Worker will attend staff meetings, attend training sessions, participate in committees and work with professional staff of other organizations that provide services to the elderly and their families.

 Responsibilities

  • Act as liaison with houses of worship with the Caregiver Program’s catchment area
  • Develop a presence in faith-based communities and provide a “safe space” for isolated caregivers to reach out
  • Identify and reach out to isolated caregivers
  • Provide outreach to other professionals
  • Provide public information on caregiving and caregiving services
  • Enhance public awareness of caregiver roles and needs
  • Develop and cultivate referral sources
  • Provide training and education to individual caregivers and to groups
  • Provide community presentations to groups of caregivers
  • Maintain complete and accurate computerized case records and statistical reports
  • Develop and maintain relationships with other agencies and resources
  • Perform other duties as required

Required

  • Minimum   BSW or BA in a related area.
  • Must be able to work Sundays plus two weekdays
  • Have an awareness of the biological, psychological, and social aspects of aging and the impact of disabilities, illnesses, and aging on the family
  • Comfort with and/or knowledge of religious organizations in Brooklyn
  • Ability to arrange and negotiate service referrals
  • Skills in establishing and sustaining interpersonal relationships
  • Ability to communicate with individuals of varying socio-economic backgrounds
  • Public speaking skills
  • Proven ability to work both independently and as part of a team
  • Basic computer literacy – Windows, Microsoft Office
  • Must be able to travel on public transportation to do home visits and community presentations

Preferred

  • Prefer bi-lingual and cultural competency in Spanish or Haitian Creole. 
  • Knowledge of community resources and public benefits eligibility
