Senior Program Manager, Behavioral Health

AIDS Foundation Chicago (AFC) mobilizes communities to create equity and justice for people living with and vulnerable to HIV or chronic conditions.  We are seeking an individual to serve as our Senior Program Manager, Behavioral Health. The Senior Program Manager will oversee programs related to behavioral health services. This includes the Road to Health and Housing (R2H), a five-year grant awarded from SAMHSA and AFC’s in-home behavioral health program. R2H project will increase the availability and ensure coordination of integrated behavioral health, primary care, housing and supportive services to improve physical health, behavioral health and housing stability for homeless adults with Serious Mental Illness (SMI) or Co-occurring Disorders (COD) in the city of Chicago. This position will manage and supervise the day-to-day operations of the project, providing clinical supervision as well as work across sectors including homeless services, health care and legal advocacy organizations. Senior Program Manager will also manage the AFC Behavioral Health Screen and ORCHID project in collaboration with Northwestern University Feinberg School of Medicine and University of Chicago School of Social Service Administration.

The Senior Program Manager is a trained clinician who will design and lead training and technical assistance efforts for Ryan White project sites. This person will provide clinical technical assistance and on-on-one support to case managers. The Senior Program Manager will work closely with agency staff to identify and address barriers to client screenings and referrals to the intervention.  The Senior Program Manager will also assist with the process of AFC/CHH becoming a Medicaid billable Behavioral Health Clinic. This includes assisting with the Medicaid application process, updating policies and procedures, and training staff. 

 

Principle functions and responsibilities of this position include the following:

Program Management

Manage relationships with partner agencies including housing, homeless services, and healthcare providers;

  • Promote the Trauma Informed Care approach within the continuum of housing and healthcare providers who work with people experiencing homelessness;
  • Support the development and achievement of team goals and related strategic plan items;
  • Responsible for Behavioral Health Program’s daily operations;
  • Ensure program policies and procedures are implemented and followed;
  • Coordinate case conferencing between project staff;

·        Provide weekly clinical supervision to Behavioral Health Specialist (2) and R2H Clinical Engagement Specialists (3);

·        Design and implement training for Ryan White Case Managers on administering the Behavioral Health Screener to Ryan White Clients;

·        Provide technical assistance to Ryan White Case Mangers throughout the implementation phase;

·        Work with Ryan White Case Managers to identify referrals for clients and assist with referral to the ORCHID intervention;

·        Assist with the application process for AFC/CHH becoming a Medicaid Billable Behavioral Health Clinic;

·        Create and monitor a program management plan for the implementation phase once Medicaid applications are approved;

·        Train Behavioral Health staff on Medicaid billing and documenting procedures.

·        Update current behavioral health policies, procedures, and documents in accordance with Medicaid compliance;

Staff Supervision

·        Hire, train, mentor, and supervise assigned staff; this includes assisting with recruitment and selection activities, scheduling and job assignments, mentoring/coaching, staff development and training, and performance management;

·        Meet regularly with assigned staff to identify priorities, discuss strategies, and review ongoing projects;

·        Provide professional development opportunities and guidance to each team member to strengthen their skill sets and job satisfaction;

·        Meet one-on-one with Ryan White Case Mangers as needed for assistance with the Behavioral Health Screen and clinical consultation.


Project Coordination

·        Coordinate projects serving households experiencing homelessness with behavioral health needs;

·        Serve as a liaison to partner agencies including University of Chicago School of Social Service Administration, Northwestern University Feinberg School of Medicine, All Chicago, Heartland Alliance Health and Legal Council for Health Justice to coordinate housing, behavioral health, health and wraparound services;

·        Utilize data for planning purposes and for system improvement;

·        Ensure a streamlined process to obtain referrals for Behavioral Health Programs and work with partner agencies to locate, assess for eligibility, and house clients in a timely manner;

·        Monitors staff compliance with reporting in information systems including HMIS, Caseworthy and Provide;

·        Create and maintain detailed project documentation and data collection plans and reports;

·        Collaborate with other Behavioral Health staff and Communications team to create and disseminate a quarterly newsletter for AFC/CHH housing case managers. 

·        Coordinate with other AFC/CHH staff and Project Management on ongoing Medicaid application and implementation process.


Quality Assurance and Data Entry

·        Complete data entry in information systems (i.e., HMIS, Caseworthy) as needed;

·        Analyze project data to provide quality and statistical reports to stakeholders;

·        Serve as liaison to Research and Evaluation team and Data Services team to ensure that data is being collected in timely and accurate manner.


Meetings and Training Facilitation

  • Attend required conference, trainings, and webinars;
  • Facilitate partnership meetings with stakeholders and leaders at partnering agencies;
  • Train service providers on Trauma Informed Care and Behavioral Health Programs;
  • Deliver presentations to key stakeholder groups;
  • Educate and train health care providers on working with people experiencing homelessness;
  • Convene meetings with project/program stakeholders to establish work plans and further initiatives;
  • Provide subject-matter guidance and mentorship to other department professionals.


Other

·        Assist with agency-wide activities as directed, including Annual Meeting, AIDS Run & Walk, and others;

·        Update job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations;

·        Protect organization's value and manage risk by keeping information confidential;

·        Perform other duties as assigned.


QUALIFICATIONS

Minimum Qualifications

Master’s degree in Psychology, Social Work, Counseling or related field

LSW/LPC, LCPC/LCSW or equivalent license

1 or more years Case Management experience

1 or more years Supervisory experience

Preferred Qualifications

1 or more years Homeless Services experience

KNOWLEDGE, SKILLS, AND ABILITIES

 

Strong case management and the ability to assess client needs create care plans, and follow-up to address barriers and ensure care is continuous and comprehensive.

Knowledge of best practice approaches to mental health and substance use treatment. This includes the ability to apply a client-centered approach that considers individual diagnoses, psychosocial assessment, and environmental factors.

Strong attention to detail and the ability to provide efficient, quality service to both internal and external clients.

Solid interpersonal skills along with the ability and willingness to respect and value the differences and perceptions of different groups/individuals.
