POSITION SUMMARY: The HIV/Health Educator will be responsible to provide HIV prevention education and treatment options, in addition to other STI awareness, to people living with HIV and people at high risk of acquiring HIV, across all Joseph P. Addabbo Family Health Centers, though primarily in and around our Red Hook, Brooklyn (site 6) and Guy Brewer Blvd/South Jamaica, Queens (site 3) locations. The HIV/Health Educators will provide services internally and via outreach. The HIV/Health Educator will work in coordination with Case Managers to meet program goals.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Knowledge of and comfort delivering sexual health education, and HIV/STI prevention strategies.
2. Able to manage behavior and conflicts effectively while remaining professional at all times.
3. Will be responsible for the transmittal of required reports.
4. Deliver sexual health education curriculums at JPAFHC sites and at external locations.
5. Establish and maintain partnerships at local schools, places of worship, NYCHA, and with other community organizations.
6. Promote patient centered health interventions via, workshops, and other educational activities.
7. Implement educational programs to help engage the patients to participate with their own health such as, Nutrition programs.
8. Educate the population on HIV prevention medications such as Prep/Pep
9. Perform HIV rapid testing.
10. Provide HIV counseling, referral, and partner notification services to clients.
11. Participate in staff meetings and staff development trainings.
12. Perform special projects and any other tasks as assigned.
MINIMUM QUALIFICATIONS:
1. High School diploma/GED. Preferred - Bachelor’s Degree.
2. Two (2) years of experience working with the HIV priority populations.
3. Good organizational skills.
4. Prior experience or training in HIV prevention/Health education.
5. Must have cultural competency and comfort working with LGBTQ populations.
6. Bi-lingual (English/Spanish) preferred.
