Case Manager – Challenge Program 

The Challenge Program is a non-profit construction training program, funded largely by the Delaware Department of Labor with a special emphasis on at-risk youth and urban revitalization.  Our main function is to provide select Delaware youth with employability training and experience in proper construction techniques and skills. 

We are seeking an enthusiastic, hard-working and highly motivated case manager to provide intensive case management and job placement assistance to our population of “high-risk” youth aged 18 – 24 years.  All services are rendered with the overreaching goal of helping our participants become highly employable, stable adults.  Our Case manager is first and foremost an advocate for our participants.  Must exercise solid boundaries and contribute positively to work environment.   Must have the ability to successfully engage and build trusting relationships with our participants.  Must be an effective multitasker, able to prioritize and meet requirements of multiple contractors under strict deadlines while managing an average case-load of 40 active participants.   Our agency is staffed by a committed, wonderful team and we achieve excellent outcomes with our trainees.  This job is equally challenging and rewarding and this is a great place to work. 

Responsibilities:

·       Performs program outreach services and represents our program at community events

·       Manages, solicits and screens all interested candidates (estimated average 250/yr)

·       Determines eligibility and handles new participant’s admission to the program

·       Develops and implements Individual Service Strategy plans outlining individual goals for each participant; then provides guidance and assistance with achieving those goals

·       Conducts case plan reviews with participants and communicates on a regular basis with other members of the service team

·       Maintains and documents monthly contact with all active participants

·       Performs job development and job placement duties, tracks and reports post placement outcomes on a monthly basis

·       Teaches and assists participants with life skills such as budgeting, banking, job searching, apartment hunting, utilizing transportation services, etc. with a hands-on approach

·       Provides supportive intervention and on-going contact with the participants and provides crisis intervention as needed

·       Makes appropriate referrals for counseling, healthcare, job training, and other services as needed

·       Assists participants with legal issues, advocates for them at court when appropriate

·       Transports clients to job fairs, college campuses, conferences, and other networking events; coordinates group life skill activities and workshops

·       Assists with creation of outcome based data collection

·       Maintains reporting standards of program, keeps well-documented records, and ensures all documentation is organized in compliance with contractual requirements

 

Qualifications: The requirements listed below are representative of the knowledge, skills, and/or abilities required at an advanced or superior level of performance.

  1. Bachelor’s Degree
  2. Experience processing federal cost-reimbursement contracts (financial, eligibility& outcome reports)
  3. Must pass drug screening and criminal background check
  4. Must possess a current Delaware driver’s license with a clean driving record
