We are currently recruiting for a Case Manager to work in our Residential Services Unit. This position will provide intensive case management, housing placement, and life skills-development services to women (many with criminal justice involvement) at WPA’s DHS-funded 38-bed shelter for homeless women.  


Responsibilities include, but are not limited to:

·      Conduct comprehensive intakes and needs assessments for clients living in WPA’s shelter for homeless women.

·      Develop service plans with clients outlining specific goals and responsibilities to achieve and retain permanent housing and self-sufficiency.

·      Administer necessary referrals to external service providers in support of client service plan goals, with special focus on substance abuse, medical/mental health treatment, vocational training and job placement.

·      Assist clients in identifying potential community housing options, submitting applications, and viewing apartments, which may include client escorts and coordination with landlords and brokers.

·      Advocate on clients’ behalf to obtain and/or maintain housing subsidies where available (Advantage NYC, Section 8, FEPS, etc.).

·      Collaborate with clients on plans to build and manage financial resources necessary to move into and retain housing.

·      Participate actively with other program staff and involved providers in case conferences supporting service coordination and goal achievement.

·      Conduct facility/safety runs and room checks to visually assess client’s needs/preferences, formulates and implements treatment, transition and discharge/continuing care planning collaboratively with individual clients. 

·      Escort clients to housing appointments and coordinate placement in permanent housing.

·      Ensure the maintenance of accurate, complete, timely and high quality client records that comply with external DHS regulatory standards and WPA agency policy and procedures. 

·      Perform any other department or agency related duties or special projects as directed by supervisor.

  • Participate in all external and internal staff trainings.


Qualifications: 

·      BA/BS/BSW strongly preferred

·      At least 2 years of documented relevant case management/direct services experience

·      Experience with NYC Department of Homeless Services and CARES system is a plus

·      Experience using computers for word processing and database entries required

·      Ability to participate in CPR training and First Aid training

·      Knowledge of government agencies, public service, Department of Homeless Services a plus

·      Bilingual Spanish/ English a plus

·      Good assessment skills

·      Excellent written and verbal skills
