The After Care Worker works rigorously with the youth from the inception of placement at the facility until after discharge to plan and prepare for reunification with family and re-entry to the community; focuses on helping the youth to make a positive transition from detention and successful re-integration with their family and community using the skills acquired during their placement; helps youth to create a discharge action plan; convenes and collaborates with individuals who will be resources for the youth; attends to the mental health and substance use treatment; using Solution Based Casework framework, can engage the family in developing plans to support family level outcomes; assist the youth in attaining educational and employment goals post discharge; monitors housing to assure stability. Other duties as assigned.
Bachelor’s degree in human service related field; must be aware of resources available within the community; able to engage families; Must possess a valid state driver’s license
An applicant for employment will not be hired if his or her driving record reflects any of the following infractions; some examples include but are not limited to:
• Suspended, revoked or otherwise invalid license
• Reckless , high risk or otherwise unsafe driving behavior within the past 3 years
• 6 or more points on license
IT IS A PLUS IF YOU:
Are Bilingual; Have knowledge of OCFS regulations and mandates; Previous experience in: community organizing, political campaigns and/or customer service ; Have prior experience with the Juvenile Justice or Prison systems.
