Project EATS (PE) seeks an experienced health professional to join its dynamic and growing team as Community Health Manager. This position reports to the PE Chief Operating Officer, and is responsible for ensuring the strength, effectiveness and impact of PE’s Farmacy and other community health programs.

 KEY RESPONSIBILITIES

 The primary responsibilities of the Community Health Manager are as follows:

 ·        Manage, improve and expand the Farmacy program to help as many patients as possible set, achieve and/or exceed their health goals, working in coordination with medical providers who partner with PE; capitalize on PE’s history of Farmacy accomplishments and its current momentum

·        Work as thought partner to the COO in envisioning and implementing a long-term plan for establishing the PE Farmacy as an effective program for patients who want to make immediate and long-term changes to their choices, behaviors and opportunities to improve their health

·        Develop and deliver (in partnership with the PE School & Community Education

Programmer) health education program content including but not limited to health & workshops, demonstrations and activities geared toward youth, families and adults

·        Solicit, cultivate and strengthen relationships with medical partners to ensure patients are aware of the PE Farmacy program and that they routinely receive Farmacy information, prescriptions, and encouragement to participate in the program

·        Maintain and report accurate and complete data, including but not limited to details of contacts, patients trying the program and repeat participants

·        Develop and implement participant, provider and program performance evaluations

·        Oversee and manage work of any direct reports (including establishing performance goals and evaluating performance) and volunteers

·        Understand grant requirements and ensure that expectations and deliverables are met

REQUIRED SKILLS & EXPERIENCE

 ·        Masters degree or five years working in nutrition, public health or community health is a plus; Bachelors degree and experience in nutrition, public health or community health required

·        Clear and effective verbal and written communicator and active and attentive listening skills

·        Experience delegating responsibility and managing others

·        Skill in planning, negotiating, problem solving, and addressing challenges resourcefully

·        Bilingual skills, especially Spanish or Chinese, is a plus
