Position Description
HIP Service Coordinator
Interfaith Works provides service coordination for clients housed in their own apartments through Montgomery County’s Housing Initiative Program (HIP). The HIP Service Coordinator conducts house visits at least once a month and provides case management services to clients. This is a full-time position.
Responsibilities:
1. Work with a Program Director and team of case managers specializing in mental health and other essential services to residential clients.
2. Meet with clients in order to assess their on-going needs and appropriate services necessary to maintain permanent housing.
3. Update client charts and files regularly, documenting every client communication. This information must be entered into a Homeless Management Information System (on-line HMIS database).
4. Meet with apartment managers and landlord to insure stability of client in the residential environment and negotiate new leases on an annual basis. Complete annual certification documents for the County.
5. Conduct home inspections once per month.
6. Provide a wide variety of client services such as referrals and resources, life skills training, health maintenance, medication monitoring, transport to essential appointments, etc.
7. Maintain communication with Montgomery County, landlords, and building management companies; and assist with required record keeping.
8. Attend HIP meetings held by the County as well as teaming meetings.
9. Attend weekly supervision and meetings.
10. Participate in Interfaith Works activities related to advocacy.
11. Other duties as assigned.
Qualifications:
1. Minimum of a Bachelor Degree in behavioral health with three years’ experience working with the target population; or a master’s degree in Social Work, Counseling, or Psychology.
2. Knowledge of principles and techniques of mental health assessments.
3. Knowledge of signs and symptoms of mental illness, substance abuse, suicidal and homicidal ideation, and developmental stages.
4. Understanding of trauma-informed care and the Housing First model.
5. Ability to work independently and with a team.
6. Must work a minimum of 40 hours per week.
7. Excellent written and oral communication skills.
8. Clean driving record mandatory (service coordinator may have to transport clients to appointments).
9. Knowledge of crisis intervention.
10. Proficiency with Microsoft Windows and Office365, in addition to Outlook, Word, Excel, and PowerPoint.
Supervision:
The HIP Service Coordinator position reports to the Program Director.
