The Adult Day Health Care (ADHC) Program at Housing Works is a unique community-based healthcare program that provides comprehensive nursing, social work, nutrition, mental health and substance use services to individuals living with chronic illnesses including HIV, behavioral health conditions, diabetes, hypertension and others. Housing Works is seeking a Registered Dietitian who is responsible for oversight of the nutritional services at our ADHC location in Lower East Side, Manhattan. The Registered Dietitian has overall responsibility for the development and monitoring of all nutritional services of the ADHC. These services include providing nutritional counseling and ongoing nutritional education to partici¬pants of the ADHC, supervising and monitoring the nutritional status of clients through written assessments, and observing food intake. Additionally, the responsibilities include menu preparation, consideration with food preparation staff, and the selection of client meals and snacks.
Completes an individual client nutritional assessment within five (5) visits and in a period not to exceed thirty (30) days after admission on all clients of ADHC, and every 90 or 180 days thereafter
Evaluates nutritional status of clients based on protocols and standards, develops individualized nutritional care plans, and maintains a nutritional record in clients’ chart.
Supervises nutritional planning, food production/preparation, dietary modifications and make all necessary adjustments on menu of the ADHC with food service staff
Contributes to initial and ongoing comprehensive care planning and attends comprehensive care planning meetings, morning rounds
Develops and facilitates group sessions in consultation with interdisciplinary team to address nutrition, menu planning, cooking, and current trends in diet therapy to clients in program
Provides in service education to the program staff and volunteers
Acts as liaison to external and internal care providers in regard to possible interaction of medical condition, drug treatment, and nutritional status
Supports and participates in advocacy efforts to end the twin crises of homelessness and AIDS
Must be a Registered Dietitian in New York State
Must have a Master’s Degree from a college/university accredited by the American Dietetic Association
Two years clinical experience in a community-based healthcare setting preferred
Experience or sensitivity to working individuals or families who have issues of mental illness, substance use, homelessness or HIV/AIDS
