Basic Functions: Has full case management responsibility for DCFS wards ages 19 to 21 preparing to transition out of the foster care system. Provides intense home based case management services to address the youth's safety, wellbeing and permanency. Assists youth in obtaining their own apartment and provides services in the client's home with the goal of developing skills for independent living upon emancipation of wardship. If ward is parenting, responsible for ensuring the safety and wellbeing of the ward's child. On call 24 hours and at times will be required to work beyond regular office hours.

Preferred Requirements: MSW or MA in a Human Service Area. Three years child welfare experience and experience working with older youth, home visiting, courts system.
Minimum Requirements: Masters in related field. One year of child welfare experience. Child Welfare Employee Licensure (CWEL) required.
Other Requirements: Must have a valid driver’s license and proof of auto insurance per agency specifications. Access to personal transportation. Must be physically able to climb up three flight of stairs.
