ESSENTIAL DUTIES AND RESPONSIBILITIES

RiseBoro Community Partnerships Food and Nutrition Access Program is an ambitious, multi-organization initiative funded by Office of Population Health (OPH) with the intent of promoting affordable food access in Brooklyn. This program installs a Food Navigator in each of Brooklyns three NYC Health + Hospitals Corporation facilities to help patients in need access affordable food and nutritional support relevant to their needs. To meet this program design, candidate in this position will require placement at one of the three Brooklyn Health and Hospital locations (Woodhull, Coney Island or Kings County). Program Director will determine location.

The position is responsible for a range of duties to ensure a successful and organized project. Core duties include the following:

  • Assist clients and assess food and nutritional barriers of individuals to determine relevant food and service referrals.
  • Communicate with clinical staff across different departments in the assigned hospital, such as the WIC, Nursing, Social Work and Case Management.
  • Support a caseload of individuals, providing referrals, follow-up contacts, and potentially collaboration with case management.
  • Serve as an ongoing liaison between hospital, RiseBoro and other external partners.
  • Maintain accurate daily logs, case notes and files for all clients.
  • Collect and enter client data into referral database.
  • Attend scheduled case review meetings, staff meetings and trainings.
  • Other relevant duties as assigned.

COMPETENCIES

  • Experience in community engagement, campaign work, or other forms of direct outreach;
  • Excellent communication, and multi-tasking skills;
  • Excellent interpersonal and relationship management skills;
  • Detail-oriented and highly organized;
  • Bilingual preferred (Haitian Creole, Spanish, or spoken Chinese);
  • Previous experience as a Community Health Worker preferred;
  • Experience working in underserved communities;
  • Genuine interest in improving community health;
  • Entrepreneurial spirit and customer-service attitude/orientation;
  • Ability to take direction as well as solve problems proactively;
  • Proficient in Microsoft Office suite; intermediate knowledge in Excel and Word.

EDUCATION EXPERIENCE

  • High School Diploma or equivalent (HSE/GED)
