WORK SCHEDULE: 24 hours per week 

SUMMARY: 

Provides information, assistance, and services for older adults that promotes activities that will help to meet their personal, social, behavioral, and emotional needs. Ability to problem solve, provide case management, and facilitate support and educational groups and advocate. Conducts home visits and outreach to home-bound elders. Provides crises management targeted to seniors at the center. Interviews, coordinates and refers participants to resources that have been identified. 

ESSENTIAL DUTIES AND RESPONSIBILITIES:  

  • Provides counseling, case management, consultation, and crisis management services at the center. When necessary, makes home visits to members. 
  • Documents and maintains all client files, case notes, monthly reports, and assists clients in an accurate and timely manner. 
  • Provides leadership in collaboration with housing providers and other senior centers to focus on housing solutions for older adults and support services as an additional prevention tool to homelessness within Ballard.  
  • Advocates for individual older adults at the system level to remove institutional barriers and minimize inequities. 
  • Facilitates existing support groups and creates new groups that respond to the needs of participants. 
  • Performs outreach activities within the center and in the community to connect elders to the community at the center and/or the larger community they live in. 
  • Provides information, assistance and referrals to assist older adults and their family members in accessing needed services. 
  • Participates in Ballard Center events as a part of the overall staff team and as agreed to with the Center Director. 
  • Contributes to the development and maintenance of a cohesive CASC staff and volunteer team, works cooperatively with and sharing in housekeeping and operational responsibilities. 
  • Demonstrates an eagerness to learn and enhances skills that promote equity and inclusion. 
  • Maintains regular and reliable attendance and punctuality at their designated work-site to support business operations, team goals and the public. 
  • Performs other duties as necessary and/or assigned.   

SUPERVISORY RESPONSIBILITY: 

This job has no supervisory responsibilities. 

RELATIONSHIPS:  

  • Supervised by the Senior Center Director. 
  • Must maintain positive working relationship with other center staff.   

QUALIFICATIONS: 

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and Experience:  

  • Bachelor's Degree in Social Work or closely related field required, Master's Degree preferred. 
  • Four (4) years of experience in a human service field, working as a social worker. 
  • Current Licensure in the State of WA is preferred and/or the ability to acquire license within two years.   

Skills and Abilities:  

  • Ability to work cooperatively as a team member. 
  • Ability to work effectively with individuals of diverse backgrounds, including African Americans, people of color, immigrants, refugees, and other culturally diverse populations. 
  • Knowledge and experience in state and federal information privacy laws, including but not limited to HIPAA. 
  • Computer experience required. Knowledge of Salesforce and other data management systems helpful.

LANGUAGE SKILLS: 

Ability to communicate clearly in English (verbal and written) with all stakeholders (staff members, practitioners and the general public). Talk, read documents and write reports and correspondence. 

MATHEMATICAL SKILLS: 

Ability to add, subtract, multiply, divide and work with budgets. 

REASONING ABILITY: 

Ability to prioritize and problem solve. 

PHYSICAL DEMANDS: 

While performing the duties of this job, the employee is regularly required to sit, talk and be able to hear. The employee is frequently required to use hands to finger, handle or feel as well as reach with hands and arms. The employee is regularly required to stand and walk. 

WORK ENVIRONMENT: 

The job is performed in a temperature regulated environment with a moderate noise level. Some work is conducted in the homes of participants with environments that can vary.
