-Home visits; assist in initial outreach and engagement efforts.
-Meet regularly with clients in homes to address concrete needs; complete necessary paperwork and then follow up by arranging appropriate services.
-Accompany clients to family court, various doctor’s appointments, Public assistance, evaluations, and schools as per service plan as needed.
-Provide toxicology tests for clients with substance abuse issues.
-Attend Clinical Diagnostic Team meetings and Supervision. If necessary attend Family team meetings and or Elevated Risk Conferences.
-Provide on-site support to Case Planners by providing child care or assisting families on site with and concrete service needs; child care occasionally during meetings.
-The position necessitates collaboration with program staff, ACS, Public Assistance, Medicaid, Housing Authority, Court, the shelter system, board of education, and other community resources.
-Work as a team with assigned Case planner for each assigned case.
-Filing out filling applications for Daycare, Housing Subsidies and furniture requests etc.

-Some evenings work required.
