The Coalition for the Homeless is the nation’s oldest advocacy and direct service organization helping homeless men, women and children. Since our inception in 1981, the Coalition has worked through litigation, public education and direct services to ensure that these goals are realized.

POSITION SUMMARY: The Client Advocacy Program provides intensive case management, support and advocacy to assist psychiatrically and physically disabled homeless New Yorkers to obtain Social Security benefits and locate permanent, supportive housing. The Bilingual Case Manager supports the work of the program through the provision direct services including advocacy, assistance in obtaining and retaining public benefits and entitlements, assistance in accessing appropriate health and mental health services, as well as completion of housing applications in English and Spanish. Services offered will be consistent with the mission and goals of the Coalition for the Homeless. The case manager will be a member of the Crisis Services team and reports to the C.A.P.  Program Director.

QUALIFICATIONS: Spanish fluency required. BA/BS in social work, human services or health discipline is strongly preferred. Prior case management experience with homeless persons, individuals with mental illnesses, individuals with a history of substance use strongly desired.  Self-starters, able to prioritize multiple tasks and work independently are also strongly preferred. 

DUTIES and RESPONSIBILITIES:

The Bilingual Case Manager shall:

1.     Provide intensive case management services to current Client Advocacy Program clients in English and Spanish. These services shall include, but are not limited to: screening and preparation of client applications for social security benefits, gathering and assessment of supportive documentation, advocacy with various systems including Social Security, the Department of Homeless Services, public assistance, as well as physical and mental health care providers, and completion of appropriate housing applications or referrals;

2.     Develop and maintain working knowledge of relevant social services systems and agencies to ensure client access to appropriate physical and mental health services, entitlements, and housing options;

3.     Accompany clients to appointments or housing interviews as necessary;

4.     Maintain accurate and timely case management notes and client records, as well as organized client files;

5.     Alert Program Director regarding client emergencies and other immediate needs and assist in efforts to promptly deliver proper services;

6.     Obtain regular (no less than weekly) supervision with the Program Director or Semior Director of Crisis Services in his/her absence to review case management plans, client issues/concerns and service needs, among other matters;

7.     Participate in weekly staff and other meetings, such as trainings and staff development and orientation activities;

8.     Participate in visits to municipal shelters to meet new clients and staff or reconnect with current clients;

9.     Assist the Program Director in maintaining contact with referral agencies;

10. Be in regular contact with partners in legal services to enlist assistance as well as to follow the progress of Social Security applications;

11. Work in conjunction with other Crisis Services programs to facilitate client services as necessary;

12. Perform all other duties as may be assigned by the Program Director and Senior Director of Crisis Services.

REPORTS TO: Program Director and Senior Director of Crisis Services

SALARY: $40K - $45K commensurate w/experience & education (excellent benefits package)
