ACS TFFC Clinician

Coordinates treatment planning and delivery for all program youth and family. Strives to build a partnership between the youth, foster family and biological family. Provides treatment in the forms of individual and family therapy to assigned youth and families.

SCHEDULE: Exempt

  • Regular hours entail Monday through Friday 9:00am to 5:00pm
  • Must be flexible to meet program needs

FUNCTIONS:

  • Provides individual therapy to program youth and family using TFFC treatment model which includes Trauma Systems Therapy
  • Completes psychosocial assessments, mental health assessments, Trauma Systems Therapy treatment plans, and participates in completion of FASP’s for program youth through the State Connections System
  • Participates in 24 hour on-call availability for program youth and parents
  • Provides record keeping and documentation for all clinical activities (case documentation)
  • Support the youths' adjustment to foster care
  • Works in collaboration with the Psychiatric Nurse Practitioner
  • Coordinate treatment-based services/supports including school, County Case Workers, Probation, Court, CHAD, and home visits, medical care, as well as other ancillary services

Required Experience:

  • Master’s Degree in social work or related field required. LMSW, LMHC preferred or eligible for Licensure
  • Experience with a strength-based, cognitive-behavioral, trauma-sensitive approach to treating troubled youth preferred
  • Experience with intensive in-home service model preferred
  • Experience treating families, children, and adolescents required
  • Experience with the administrative components of treatment (case documentation) preferred
  • Experience with completing comprehensive mental health assessments and treatment plans required
  • Experience or familiarity with Connections documentation a plus
  • Valid NYS Driver’s License
