Position: Community Resource Coordinator


Reports to: Chief Officer of Youth and Family Services


Classification: Full-time/Exempt

Position Statement:


The Community Resource Coordinator (CRC) is responsible for supporting participants across all three KHCC departments as a means of moving towards seamless service provision. The CRC will act as a central resource to various intake staff from our Early Childhood, Youth, Clinical, and Adult programs when families present with needs that span multiple areas at KHCC. She/he will accomplish this through the support of social work interns, utilization of our new agency wide database, education/training with all KHCC staff about the importance of and procedures for intra-agency referral, coordination of groups and events that may benefit all KHCC adults, streamlining of outside referrals, and the development of an expanded partnership network. The CRC is a member of the KHCC Leadership team.


Duties and Responsibilities:


  • Develop intra-agency referral system to more easily allow participants to access multiple services within KHCC. Utilize the new KHCC database, universal intake form, and universal screening to be filled out by adult participants or parents of our child and youth participants.


  • Provide consistent case management, advocacy, and outside referrals to families who present with needs beyond the scope of KHCC’s service continuum.


  • Spearhead Parenting Journey group offerings for participants across KHCC programs; facilitate groups with support of new and existing staff and interns trained in Parenting Journey group curriculum.


  • Supervise two (2) BSW or MSW interns from local schools of Social Work.

  • Coordinate workshops and events that may benefit adult participants and/or families from any KHCC program, with a specific focus on newly arrived immigrant participants as needs center around immigration status, legal obstacles, etc.

  • Expand our community partnership network by strengthening existing connections and developing links to new agencies.


  • Solidify individual program referral sources and/or partnership contacts into one KHCC Community Resource Guide

  • Evaluate progress towards cross agency service provision via staff feedback at monthly leadership meetings, by utilizing dashboard function on agency database, and through the creation and implementation of staff and participant survey tools designed to capture information about the move toward more cohesive work with KHCC families.


Requirements:

  • Bilingual Spanish required


  • Bachelor’s degree in human services related field required; BSW/MSW preferred with ability to supervise social work interns.


  • Flexibility with schedule to accommodate collaboration across diverse program areas with varying program hours.


  • Exceptional organizational, follow-through, and time management skills; Comfort and familiarity with multiple computer platforms and database systems a plus.
