The Prevention Program provides comprehensive home and community-based preventive services to children and their families. The program will provide services supported by the evidence-based model, Mobility Mentoring. The Program model includes working from a coaching model wherein the interdisciplinary team works closely with families in the community to establish and accomplish family centered goals.  


As the Case Aide, you will provide a variety of supportive casework services for both Case Planners and children and families referred by the Administration for Children Services (ACS), or self/community providers who are deemed to be at risk of foster care placement. In this role, you will be involved with retrieving educational, medical, mental health, and other relevant documents from various providers and will be providing advocacy/referral assistance to client-families as needed.


What You Will Be Doing: 

• Provide clerical assistance to casework staff, Program Supervisor, and Director

• Perform Family Home Assessments

• Monitor visits in the absence of the casework and supervisory staff

• Provide escort service for parents and children as assigned

• Document all visits and collateral contacts

• Flexible hours including Saturdays as needed

• Assist Case Planners with family related service needs

• Participate in program meetings and supervision

• Ensure that entries into connections & PROMIS are updated, accurate, and submitted within deadline

• Assist in filling charts and ensuring charts are accurate and updated

• Other tasks as assigned by the Program Supervisor


What We Are Looking For: 

• You have a HS Diploma or High School Equivalency (HSE), Bachelor’s degree in Social Work or related field preferred 

• You have prior experience in child welfare or other social services, including day care, youth services, or other related programs that involve direct work and contact with children and families 

• You are passionate about accuracy and meet deadlines on time all the time.  

• You thrive in a busy environment and able to shift gears when needed.  

• You have a flexible schedule- able to work weekends and evening when required.

• Ability to work independently, as well as contribute to a team

• Strong communication skills and possess computer skills and willing to learn new systems 

• Bilingual Spanish/English fluency is a strong plus!

• You are willing to be an advocate for your program, participants and community. 

• You have a strong commitment to advancing diversity, equity, and inclusion.
