    Position Title:           Medical Case Manager - Harbor Project

    Reports to:             Case Management Coordinator

    Department:            Direct Services

    Program:              Case Management / Client Services

    Location:              Pathways To Housing Inc.

    Status:                Full Time, Non-Exempt, Eligible for benefits

    Qualifications:           

    • Bachelor’s degree in social work, psychology or related field required and Addiction Counseling Certification a plus. Master’s degree preferred; Experience in social/human service delivery mental health and addiction counseling. 
    • Demonstrated expertise in case management, supportive housing and client advocacy. 
    • Ability to work with diverse populations including clients and staff in critical and emergency situations. 
    • Sensitive to cultural, economic, gender, and sexual orientation differences. Patience, creativity, flexibility and compassion to people with disabilities. 
    • Knowledge of local resources and service pool systems. Understanding of medical and psychosocial issues related to HIV disease, Mental Health, Drug and Alcohol abuse and other related conditions. 
    • Flexibility in work sites, work hours, including occasional evenings and weekends. Valid driver’s license required.

    Basic Function:          

    • Provide a full range of HIV case management services to 20 intensive need clients. 
    • Function as a part of the Direct Services team to ensure that high quality services are provided to clients. 
    • Work in a responsible manner to assist formerly homeless individuals who are living with HIV/AIDS, psychiatric disabilities and/or substance abuse.

    Specific Responsibilities:

    • Complete Intake in accordance with Pathways Intake and Assessment procedure.
    • Participate in on-call rotation.
    • Perform face-to-face needs assessments with clients, obtaining medical, demographic, and consent information.
    • Assemble service care plans together with client and other members of support team, while ensuring confidentiality.
    • Provide case management and advocacy by carrying a caseload of persons symptomatic with HIV disease, Substance abuse and Mental Health histories.
    • Available for bi-monthly home visits, as well as accompany clients to medical appointment and treatment facilities visits as appropriate.
    • Outstationed assignment at Pathways to Housing Inc.
    • Assist clients in advocating and negotiating social services arranging and ensuring linkage with community resources.
    • Provide information and referral, including bi-monthly follow-up and monitoring of related activities, and reassessment as needed.
    • Provide information and education to members of clients’ support system as necessary when informed consent has been received.
    • Provide individually tailored client driven services to each client—i.e. housing placement, independent living skills, vocational guidance, integrated substance use treatment, and counseling
    • Consult with care providers in discharge planning and home care plans.
    • Consult with Action Wellness and Pathways Direct Service staff, volunteers, therapist and medical staff regarding specific clients’ plans, as appropriate.
    • Participate in some activities outside of normal business hours
    • Provide intake coverage as assigned.

    Performance Standards:

    The following performance standards are specific to this position. Meeting or exceeding these standards is an expectation of employment, as is in compliance with all organizational and departmental policies, procedures and expectations.

    • Maintain awareness and knowledge of resources, as well as benefits and entitlements, necessary to appropriately refer clients for services and advocate for client needs.
    • Complete reports as determined by job function and department.
    • Provide leadership and administrative and clinical back-up coverage for team members
    • Interface with outside collaborators and function as a liaison as dictated by specific job function to assist in coordination of services and procedures for clients to access services.
    • Adherence to clinical and procedural policies in the Action Wellness Direct Services Manual
    • Attend Action Wellness’ Direct Services, In-services, and General Staff meetings.
    • Attend scheduled meetings, e.g. team rounds at least once per week, daily clinical case conferences and weekly individual and monthly group supervisory meetings
    • Maintain accurate, timely records as required.
    • Document the minimum contact with each client assigned to caseload per time frame outlined in the Direct Service Policy and Procedure Manual.

    Benefits

    Accrual Vacation Leave: is accrued on a semi-monthly basis. Accrual of time is suspended during a leave of absence. Vacation Leave and Extended Sick Leave, also called ESL, must be accrued before it is used. Vacation Leave hours accrue each month based on length of service. Full-time staff members accrue: • 80 hours during their first year of employment. • 120 hours during their second year of employment. • 160 hours during their third year of employment. • 200 hours during their fourth year of employment, and each year thereafter. Part-time staff members accrue Vacation Leave on a prorated basis.

    Sick Leave Account: The Philadelphia Sick Leave Ordinance states a staff member accrues 1 hour of Sick Leave for every 40 hours worked, earning a maximum of 40 hours for the year; however, the agency issues Sick Leave in advance at the full amount (40 hours) on July 1st or on hire date, to staff members to use during the year.

    Extended Sick Leave Account: Extended Sick Leave Account (ESL) provides additional paid time off for staff while they are on short or long-term disability. ESL is an agency benefit that is offered and is not mandated by the Department of Labor. Full-time staff members at Action Wellness earn 4 hours per month of Extended Sick Leave (ESL). Part-time staff members accrue ESL on a prorated basis. ESL balance accrues to a maximum of 500 hours. ESL may not be used for intermittent leave. ESL can be accessed to supplement Short or Long Term Disability payments, which are 60% of a staff member’s pay. Unused accrued ESL will not be paid out at time of employment separation. Vacation Leave and Extended Sick Leave will not accrue during periods of short and long term disability.

    Holidays: The following constitute paid annual holidays recognized by the agency: • Martin Luther King Day • President’s Day • Memorial Day • Juneteenth • Independence Day • Labor Day • Thanksgiving Day • The Day After Thanksgiving • Christmas Eve (1/2 Day) • Christmas Day • New Year’s Eve (1/2 Day) • New Year’s Day as well as summer Fridays (Memorial Day through Labor Day) we close at 3:00pm instead of 5:00pm.

    403B Retirement plan: We have a 403B plan (Pre-tax or ROTH option). After working for 1 year we match 50% up to 6% as the budget allows. For example, if you put in 4% the agency will match that with 2%, if you put in 6% the agency will match that with 3%, and if you put in 10% the agency will match that with 3% if the budget permits.
