Brain Injury Services is looking for an individual with strong organizational and group facilitation skills to lead our community of survivors. The goal of the Community Coordinator is to provide innovative programming and facilitation of the day program directly, and through support of the part time Activity Coordinator role, under the general direction of the Program Manager.

Under general supervision of the Program Manger, the Community Coordinator is a Full time position, responsible for the implementation and development of day program activities for persons with brain injuries and ensures that all activities planned are done so to help participants achieve their individual goals and the organizational mission. The auxiliary goal of the Community Coordinator is to provide intensive independent living skills support sessions for a limited number of community-based clients on a flexible monthly basis      

 

ESSENTIAL DUTIES AND RESPONSIBILITIES 

  • Ensure that all program activities are based on the needs of the participants and are designed to help them achieve their personal goals.
  • In collaboration with the Activity Coordinator and Program Manager to develop, implement and coordinate all program activities based upon the needs and interests of the members
  • Coordinate activities with existing and new community agencies on a monthly basis
  • Maintain up to 10 program client records as designated staff contact
  • Complete and store monthly documentation packets for planned activities
  • Maintain up to date client files with monthly updates
  • Complete all weekly maintenance related duties with planned activities
  • Complete all weekly supply planning and pick-up duties as assigned
  • Complete all assigned monthly mailing documents by assigned deadlines
  • Complete all Program Tour, related case management system documentation, and admissions packet documents with clients as assigned
  • Facilitate all community-based planned activities on a weekly basis
  • Assist in the implementation of Vocation Transition Program (VTP) activities as needed
  • implement behavior interventions based on positive behavioral support methodology as needed
  • Participate in staff development activities, meetings, and program conferences as needed
  • Assist with regular program evaluation and quality improvement initiatives
  • Assist with fundraising activities
  • Provide at least a monthly living skills support session for each active community client
  • Create and facilitate living skills support goal plans for a limited time period: defined as less than 1-year completion date for all plan objectives
  • Maintain up to date community-based client files with monthly updates in the BIFirst Case Management system and monthly program data reports 
  • Current CPR and First Aid Certification, StarCraft Vehicle Operations Training, and ServSafe Food Protection Manager Certification
  • Other Duties as assigned

 

QUALIFICATIONS, KNOWLEDGE SKILL REQUIRED

A minimum of a Bachelor’s Degree in special education, rehabilitation counseling, social work or a closely related field is preferred. A minimum of one-year experience providing community-based services to persons with disabilities, preferably those with brain injuries, is highly desirable. Preference will be given to those applicants with experience in community-based day programs, and knowledge of person-centered practices.

 

Knowledge of:

·      Medical and psycho-social aspects of brain injury

·      ADA and other laws/regulations that promote and/or protect the rights of persons with disabilities

·      Brain injury         


Skills in:

·      Effective verbal and written communication with clients and support personnel

·      Identifying and developing interagency relationships

·      Identifying the relevant documentation necessary to support a brain injury

 

Ability to:

·      Engage and manage several diverse activities in an average work week

·      Communicate effectively with diverse groups or individuals

·      Engage in moderate physical activity as needed

·      Ability to maintain a safe driving record and operate a 12 passenger vehicle 

 

 ADA SPECIFICATIONS This job operates in a professional office environment and at various community sites. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. This job requires an ability to lift a minimum of 40 lbs and the following related activities: bending, kneeling, and using small to large hand tools. Frequent travel within the service area is required as well as the ability to drive a passenger vehicle.
