Responsible for connecting clients with services, including but not limited to healthcare, insurance, social services, and other community resources. Assist clients by coordinating appointments, making referrals, and assisting with supportive services as needed. Provide case management and education to individuals.


  • Conduct neighborhood outreach and networking to connect with high need individuals
  • Increase health awareness and recruit clients at health fairs and other community events
  • Develop relationships with organizations to receive referrals and offer educational seminars
  • Identify client needs, goals, strengths and challenges
  • Provide comprehensive case management services that include referring and providing direct 1:1 assistance to help clients obtain and consistently utilize prenatal care, preconception/interconception care, primary care, and other needed community services.
  • Assist clients with completing applications, finding service providers, scheduling appointments, and arranging transportation/child care.
  • Provide social support to encourage and reinforce positive choices and behaviors
  • Provide education about available family planning services in the community
  • Conduct psycho-social assessments and provide referrals to behavioral health services
  • Carry a caseload of clients
  • Collect data and maintain confidential client files
  • Make home visits, as necessary
  • Participate in weekly case conferences and submit monthly and quarterly reports
  • Handles other assignments as requested


  • BS in Community Health or related field
  • Computer literacy in MS Office Suite
  • Experience providing outreach to women
  • High degree of familiarity with Essex County health and social services required
  • Bilingual (English/Spanish or English/Creole) preferred
  • Good verbal and written communication skills
