Care Manager Residential Programs

Position description:

The Care Manager for Residential Programs works with resident families to provide resident-centered, strengths-based support, case management, advocacy, risk management, and safety planning. The Family Crisis Center offers emergency shelter and transitional housing to individuals and their children who are experiencing domestic violence and in need of safe housing. Residents are assisted in making progress towards self-identified goals and more stable and permanent housing. The Care Manager provides direct client services and will supervise additional case workers. This position reports directly to the Shelter Director. This is a full-time position, Monday through Friday, with 2 evenings required

Responsibilities:

Working with the full residential program team provides the following core services:

• Establishes initial contact with new residents within 24 hours or 1 business day

• Provides initial assessment of all incoming resident, including strengths-based inventory, housing barriers, family strengths assessment, and other tools

• Works with residents to assess risk and formulate safety plan to reduce risks

• Works with residents regarding children and transitional school plans • Works with residents to obtain necessary documents

• Assists residents in applying for, transferring, or maintaining public benefits, such as public cash assistance, SNAP, LIEAP, and medical assistance

• Engages residents in developing goals and creating a service plan for the shelter stay

• Schedules and leads regularly scheduled progress meeting to review assessment and plan

• Reviews client record for completion of paperwork and record

• Coordinates assignment of staff to provide ongoing case management and facilitates transition

• Carries an active caseload of residents for ongoing case management; meets 1-3 times a week with residents to focus on engagement, facilitate problem-solving, and review progress towards goals, including milestones towards housing

• Provides crisis management and de-escalation within the shelter setting, including client mediation and resolution of conflicts; conducts house meetings

• Assists residents in identifying and practicing coping strategies and works with shelter monitors to address problems arising in shelter living

• Builds service partnerships, resources, and linkages with related community-based organizations

• Provides appropriate referrals and assists clients in accessing services and addressing barriers

• Participates in data entry and collection, documentation, outcome evaluation, and reporting

• Participates in staff meetings, individual and group supervision, trainings, and other meetings as assigned

• Assists in the daily operations of the work site to maintain adequate coverage

• Maintains client privacy and confidentiality

• Other duties as assigned

Minimum education required: Bachelor’s degree in social work or a related field, or equivalent training and experience.

Minimum experience required: 2 years previous experience in case management required. Prior experience in domestic violence agency is preferred. Previous supervisory experience is preferred.

Physical Requirements and Working Environment:

• While performing the duties of this job, the employee is regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and telephone keyboard reach, stoop kneel to install computer equipment

• Light to moderate lifting is required

• Regular, predictable attendance is required

• Tolerates moderate noise (residents talking, children playing, phones ringing)

• Ability to maintain composure in shelter work environment

Must have valid:

• Criminal background check

• State issued identification

• Professional references
