    The Family Center is seeking a Full-Time Program Coordinator/Physician Assistant, who will provide integrated, person-centered Substance Use Disorder (SUD) and Co-Occurring Disorder (COD) treatment including assistance with managing acute and chronic health conditions, assessment for SUD and COD treatment readiness, coordination of care with other Healthy Branches team members and outside organizations, including housing and employment assistance providers. 

    The Program Coordinator/Physician Assistant assesses all potential Healthy Branches consumers for enrollment in services and leads the team in initial treatment planning and ongoing monitoring of consumers’ treatment plan goals.  This position will work closely with all team members to ensure all consumers are tested for HIV and Hepatitis C, and receive vaccination and follow up as needed.

    The Program Coordinator/Physician Assistant will receive supervision and training

    related to medical care from the Medical Director. This role reports to the

    Deputy Executive Director for Behavioral Health and Support Services for

    project management responsibilities. 

    Primary Responsibilities

    •Supervise the Healthy Branches team, which includes Care Managers, CASACs & Peer

    Specialists

    •Provide leadership and guidance to the Healthy Branches team to ensure service

    integration needed to meet the complex care needs of individual consumers

    •Assess all potential clients for SUD and COD treatment readiness and lead team in

    initial treatment planning and ongoing monitoring of each consumer’s treatment

    plan goals

    •Provide HIV and Hepatitis C testing and follow up including PrEP, risk-reduction

    counseling, partner testing and follow-up, as well as linkage to confirmatory

    testing and to HIV and Hepatitis treatment as appropriate

    •Provide training for team members and all agency staff on HIV and Hepatitis C testing,

    diagnosis, and treatment and the importance of ongoing testing and follow up

    for all consumers at high-risk of exposure

    •Prescribe medication for SUD and COD treatment and provide medical consultation for all

    Healthy Branches consumers, including monitoring health indicators and ongoing

    care coordination for acute and chronic health conditions, including HIV and

    Hepatitis C

    •Ensure crisis management and diversion prevention plans are up-to-date for all

    consumers

    •Maintain contracted levels of service and ensure the clinic's ability to meet enrollment

    and retention targets

    •Ensure timely collection and submission of data required by funders and regulatory

    oversight agencies

    •Other duties as assigned

    Required Qualifications:

    •PA licensed in New York State

    •minimum 5 years of experience as a PA in SUD/COD treatment

    setting

    •Management experience in a clinical setting, including team

    leadership/supervision

    •Experience working with HIV+ individuals  

    •Strong leadership, excellent organizational and communication

    skills

    •A track record of meeting compliance standards

    •Ability to multitask and to work and function under pressure

    Location: This position is eligible for hybrid work. Our office is located at 493 Nostrand Avenue, Brooklyn, NY.

    Starting Pay Range: $125,000.00-$130,000.00 per year

    Schedule: Full-time, exempt
