Therapeutic Case Manager (Relocation)

Role and Responsibilities

The duties listed below are intended as examples of the types of work that may be performed by the Case Manager but are not all necessarily performed and do not include all specific functions and responsibilities that the Case Manager may be expected to perform.

  • Assist clients in locating suitable housing in local and distant communities.
  • Assist client in locating community resources in new locations.
  • Performs outreach to facilities and other appropriate agencies regarding relocation services.
  • Works with lead agency (DHSS) to arrange details of waiver services.
  • Meets regularly with Clients to discuss progress, problems, and plans.
  • Take clients to potential sites for tours.
  • Assist clients in completing housing application.
  • Develops an individualized person-centered transition plan.
  • Complete necessary paperwork to apply for Emergency Assistance, cash, food support, 1st month’s rent deposit, utility bills, etc.
  • Works with clients to safety plan in those cases where relocation is undesirable or not feasible.

Qualifications and Education Requirements

· Bachelor’s degree in Social Work or Human Services or related field with experience in Case Management or a two-year degree in Human Services with at least two years of paid work experience; Master’s degree encouraged.

·  Knowledge of social service programs and operations; laws and ethics pertaining to data privacy.

· Knowledge of socio-economic factors that promote stable family life.

· Possess the ability to utilize effective interviewing and supportive counseling techniques; to analyze complex situations and evaluate potential for improvement; to work independently and follow directions and guidelines; to seek out resources and information; to establish and maintain accurate records; to effectively communicate verbally and in writing; and to multi-task and effectively manage work at different sites.

· Being able to work with a diverse population, including but not limited to individuals with limited income and individuals with substance abuse disorders, mental illness, and physical disabilities.

· Work as a team with staff of diverse background and cultures.

·  Must have a strong work ethic, a positive attitude, and a good sense of humor.

·  Must express a basic belief that people can change in positive ways.

· Computer literacy required (ability to compose and send emails, draft client case notes, use internet-based database system, compose documents in Microsoft Word, and perform basic internet research).
