The Bridge, an innovative and well respected mental health agency serving adults with serious mental illness, substance abuse, co-occurring mental health and substance abuse disorders, and co-morbid medical conditions is a Field Coordinator to supervise delivery of services to clients, including admission to program, ongoing rehabilitative services and counseling, and discharge from program; available for 24-hour crisis management via phone and in person during certain situations; directly supervise Case Managers in the field; train staff on field work and proper documentation according to agency policy; train staff on CPR and First Aid (obtain certification and “train the trainer” status); responsible for coordinating and executing client move ins and move outs in a timely fashion, including facilitating apartment turn over. In conjunction with the Facilities Department, manages vendors in the field, submitting invoices and confirmation of services; attend housing court and assists Property Management with client legal cases if applicable; escort clients to treatment or benefit appointments when needed; conduct home visits for clients that are high risk or in crisis; prepare programs for site visits from funders including auditing charts; maintain a small client caseload when needed; work in conjunction with facilities, landlords, superintends and administrative staff; reinforce department objectives and continuous quality improvement efforts within the agency; ongoing assessment of building maintenance requirements and apartment inspections; represent the Agency at meetings, conferences and training as required. Any other duties as may be assigned.


Associates degree with related experience

Experience working with mental health, crisis intervention and managing trauma triggered behavior. Excellent time management skills; stress resistant, flexible; team player; open to feedback; creative; excellent organizational skills; willingness to perform CPR/First Aid/AED/CPI/Naloxone. Experience supervising staff in the field; strong interpersonal communication and organizational skills. Preferred knowledge of AWARDS. Spanish speaking a plus.
