Job Description:
For hundreds of families, the Issaquah Food & Clothing Bank is the first place they visit when they need a little extra help. We not only provide our community with basic needs - food, clothing and assistance accessing services and resources - we provide heart, hope and comfort.
We have partnered with Eastside Fire & Rescue to create a unique Mobile Integrated Health Program (MIH), called C.O.R.E. Connect (information on Seattle’s MIH program). Most MIH Programs have a social worker on staff at the fire department, but our joint program will have a social worker on staff at the Issaquah Food & Clothing Bank responding to referrals and calls with Eastside Fire & Rescue staff.
The Case Manager will respond to and provide care coordination services for residents with mental health, medical, behavioral health, substance use, housing, and other social service needs. The position includes considerable public contact, and will often include directly contacting and/or accompanying emergency responders, law enforcement, and other health and social service providers to assist in accessing mental health, substance use, behavioral health, housing, medical and other social services.
While the majority of this position is working with Eastside Fire & Rescue, this position is part of a small, dynamic staff at Issaquah Food & Clothing Bank and may occasionally require working in the building on general operations, including occasional evenings and weekends.

STATUS: Full Time – 40 hours per week. Benefits eligible
PAY: $24-26/hr DOE

REPORTS TO: Community and Social Services Manager
JOB SUMMARY:
Work closely with Eastside Fire & Rescue staff and assigned CORE Connect firefighters to schedule and perform home visits. Attend trainings and meetings as needed or helpful to perform job duties. Maintain and develop relationships with partner organizations to make quality referrals.
DUTIES AND RESPONSIBILITIES:
1. Connect and build relationships with a broad range of community members, including underserved populations.
3. Track, organize and analyze program data for reporting as needed.
4. Attend community meetings and trainings to network and gain skills as needed.
5. Develop and maintain positive, collaborative working relationships with clients and other community organizations.
6. Act as an ambassador for the Issaquah Food & Clothing Bank and Eastside Fire and Rescue
7. Participate in disaster or emergency-related duties, as needed.
8. Provide support to fellow department team members, as well as outside departments, as needed.
10. Perform other duties as assigned by supervisor.
