Kaleidoscope Inc, a non-profit, child welfare organization located in Chicago, Illinois is looking to fill the position of Stabilization Coordinator for our Enhanced Intensive Placement Stabilization Pilot Program.

General Description

The Stabilization Coordinator is responsible for working with assigned foster care placement agencies in order to help stabilize youth who have psychiatric needs (recent placement in a psychiatric hospital for care). The Stabilization Coordinator facilitates the child and family team meeting process, in order to improve assessment, engagement, and planning for children and their families. The Stabilization Coordinator aims to improve delivery of services by identifying barriers to optimal and timely care.

Essential Duties and Responsibilities

Position-Specific

  • Demonstrates agency clinical philosophy in all aspects of program activities.
  • Promptly responds to referrals by reviewing referral information and facilitating initial meetings with assigned agencies to assess current and future needs of youth, families and assigned agencies.
  • Coordinates initial child and family team meetings within 30 days of referrals. Works with lead agencies to coordinate subsequent child and family team meetings, as needed.
  • Facilitates consistent and ongoing communication with assigned agencies to identify family and child needs and system barriers.
  • Provides training and support to the caregivers on trauma, addressing trauma behavioral symptoms and the promotion of healthy and secure attachments.
  • Collaborates with child and family teams to develop, review and modify personalized action plans, outlining objectives for children, caregivers, assigned agencies, and other collaterals as needed.
  • Ensures documentation of all admission, ongoing and closing activities in a timely manner using approved forms, including, but not limited to CANS.
  • Consults regularly with supervisor, DCFS liaison, and other service entities to ensure mental health, permanency, safety and wellbeing needs of youth and families are being met.
  • Collects and reports case-specific and program summary outcome data, in collaboration with the agency’s PQI department.
  • Demonstrates a working knowledge of Enhanced IPS Pilot Program requirements, COA accreditation standards and related DCFS policies and procedures.
  • Completes other duties as assigned by the IPS Program Manager.

Level of Language Proficiency

Educational Requirements/General Qualifications

  • Minimum Degree Required: Master's degree in social work or related field
  • Minimum Years of Experience: At least 3 years of social service or lated experience (must include 2 years in child welfare)
  • Licensure: LCSW or LCPC licensure preferred
  • Other Requirements: Must possess or pass DCFS CWEL, CANS , CERAP and other State of Illinois exams, as required. Must have prior experience in faciliatating child and family team meetings. Excellent interpersonal, oral and written communication and time management skills. Full time use of insured, operting automobile and valid drivers license.
