Internal Job Title: Door-to-Door Assessment Social Worker

Department: Constituent and Community Services 

Reports to: Manager, Door to Door 

Status: Full time,  non-Exempt

Position Summary: DOROT’s Social Workers provide case assistance and support to a broad spectrum of active as well as isolated and homebound seniors. This includes assessing seniors for all of DOROT’s Programs. The Door-to-Door Assessment Social Worker will assume the very important primary responsibility of conducting comprehensive, psychosocial phone or in-home assessments of new clients and follow-up assessments with ongoing clients, with a primary focus on those individuals served by the Door-to-Door Program. Follow up assessments are imperative to confirm a client’s ongoing appropriateness for program attendance or need for additional services.

Primary Responsibilities:

  • Conduct phone and in-home assessments (both first time and reassessment) a minimum of four to five full days a week, working independently in the community. This includes: 
  • Providing short-term case assistance, information, referral, and advocacy to these seniors and their caregivers following the assessment or as needs arise; and  
  • Collaborating with the social work team and other staff to integrate these seniors into DOROT’s wide array of programs, with a special emphasis on engagement in the Door-to-Door Program. 
  • Collect, track, and maintain case assessment and related client interactions using DOROTs’ online database including routine assessment data entry and the review of monthly statistics for internal needs and reports to funders. 
  • Play a role in the analysis of Door-to-Door and other program/project impact, helping to identify gaps and develop recommendations for program improvement. 
  • Provide support to and participate in DOROT’s community-wide intergenerational volunteer programs and on-site events, schedule permitting.  
  • Staff the Information and Referral (I&R) telephone line and/or Kosher Meals at Home program, as needed.
  • Address special projects as assigned by Manager.

Qualifications:

  • LMSW required 
  • 0-3 years of professional work experience 
  • An affinity for working with an elderly population 
  • Strong clinical assessment skills  
  • Strong written and verbal communication skills 
  • Strong organizational skills 
  • Ability to work independently and collaboratively as part of a clinical team  
  • Knowledge of aging and community resources  
  • Proficiency with Office 365  
  • Experience working with volunteers a plus 
  • Fluency in Spanish a plus  
  • Availability to work a minimum of six Sunday and/or evening events per year, which includes DOROT’s signature Package Deliveries.
