Program Background:

Collaborative Care/IMPACT is an evidence-based interdisciplinary, integrated model of care that provides treatment for depression and anxiety in a primary care setting. Collaborative Care uses an approach that is 1) team-driven, 2) population-focused, 3) measurement driven, and 4) evidence-based. The team consists of a primary care provider, care manager, and consulting psychiatrist who coordinate their care so that all team members work at the top of their license to provide treatment (both psychopharmacological and evidence-based therapy) to a defined population. Treatment is consistently measured to determine effectiveness based on patient self-report, with a goal of managing symptoms and reducing scores on self-reported, validated treatment tools. As the most-studied primary care based behavioral health intervention, Collaborative Care has proved adept at reducing depression and anxiety in individual patients and the population in an outpatient setting, and reducing overall cost to the healthcare system.

Job Description:

The Care Manager Position will provide and coordinate behavioral health care management for patients in Collaborative Care (integrated primary care/behavioral health) in an adult outpatient medical clinic of Wyckoff Heights Medical Center. The individual will be responsible for a wide range of duties including conducting mental health evaluations of patients, providing psychoeducation to patients on treatment options for behavioral health issues, consulting with patients and primary care providers about treatment options and preferences, coordinate the initiation of the treatment plan, close monitoring of patients using standardized measures, provision of brief, structured psychotherapy and behavioral activation to address symptoms of depression and anxiety, participation in regular caseload supervision with consulting psychiatrist/psych NP, focusing on patients not adequately improved within specified time frame, coordination of communication between patient, primary care provider, and consulting psychiatrist/psych NP, and providing recommendations for change in treatment according to evidence-based algorithms and discuss recommendations with the primary care provider.

The Care Manager will also be responsible for facilitating formal and informal teaching and consultation to other health professionals, participation in quality improvement activities, maintain an electronic registry of all patients participating in Collaborative Care, and consistent and thorough documentation in the eMR, e-Clinical Works. The ideal candidate will have experience in an outpatient clinical setting serving adolescents and their families, a background of providing brief, evidence-based treatment, and will demonstrate cultural sensitivity and excellent communication and diplomacy skills to the diverse patients population and staff at Wyckoff.

Position Requirements:

Essential qualifications and competencies:

  • Licensed Psychologist (Ph.D./Psy.D) or Licensed Clinical Social Worker (LCSW), licensed in NY State with current registration
  • Bilingual (English/Spanish) preferred
  • Minimum 2 years’ experience in a relevant setting
  • Prior experience in brief, structured, evidence-based psychotherapy
  • Ability to work with high degree of independence in a fast-paced environment
  • Proficiency in Microsoft Excel
  • Competency in DSM 5 diagnosis, ICD-10, and use of CPT codes
