The nurse will assess and track client health needs in an effort to provide intervention with an emphasis on homeless persons with AIDS/HIV.  Provides health assessments, education, and referrals and monitors those homeless persons with AIDS/HIV, mental illness, chemical dependencies, diabetes and other serious health conditions; and, communicates with health facilities regarding the status of residents; communicates with staff, volunteers, and interns and outside agencies and health representatives as needed to serve the residents.

ESSENTIAL DUTIES

·        Assesses clients health needs

·        Ensures that opportunistic infections and medication side effects are identified and treated

·        Educates clients, staff and volunteers on health issues

·        Serves as a resource on how to decrease and avoid engaging in risky behavior effecting clients health

·        Links clients to appropriate health facilities

·        Meets with staff to plan client care

·        Monitors medication review and other health related staff functions

·        Fills pill boxes, and helps assure medications are filled

·        Works as part of the staff at in a team approach PLASE; Attends necessary meetings;

·        Monitors use of medication whether self-monitored, or monitored by PLASE or prescribing clinic

·        Plans trainings for staff (i.e. medication monitoring and health issues)

·        Coordinates with health facilities on behalf of clients

·        Meets with residents, family members, staff and other appropriate caregivers to inform, educate and plan for the clients’ needs

·        Advocates for clients medical and psychological treatment

·        Completes reports, forms and correspondences as necessary

·        Provides crisis and emergency care

·        Performs other duties as needed

CERTIFICATIONS AND LICENSES

Applicants must have a valid license to practice nursing in the State of Maryland

Applicants must have a valid drivers’ license to operate a vehicle in the State of Maryland.

PHYSICAL AND ENVIRONMENT CONDITIONS

Work requires no unusual demand for physical effort. 

Work environment involves everyday risks or discomforts which require normal safety precautions typical of such places as offices, meeting and training rooms, residences or vehicles (e.g. use of safe work place practices: avoidance of trips and falls, observance of fire regulations and safely operating a motor vehicle).
