DESCRIPTION

The Community Health Worker (CHW) works in cooperation and collaboration with Bradley Free Clinic, Carilion Clinic’s Emergency Department and other community partners to ensure Spanish-speaking patients in the Roanoke Valley and surrounding areas have access to primary care and other healthcare services. 

This is a pilot program for three months beginning June 1, 2021 for a CHW to work in the community of Roanoke and surrounding areas. There is an opportunity for the position to extend beyond the initial pilot. 

The CHW is responsible for supporting all aspects of patient care and access, meeting with patients in the ED, coordinating appointments at Bradley Free Clinic or other primary care sites, meeting with community partners to build relationships within the Spanish speaking community, and following patients throughout the process. This position will also serve as language access and/or clinical support at Bradley Free Clinic as needed.

Ideal candidates will be fluent in Spanish (required), have clinical experience (CMA or similar certification preferred), and be passionate about public health and improving access to health care services. Candidates should be comfortable using Microsoft Office, writing and speaking in Spanish, communicating professionally by email, and performing date entry and database management. This is a grant funded position, so it is essential the ideal candidate is hard-working, self- motivated, outgoing, works independently, and is detail oriented.

DUTIES

Patient Communication and Coordination

  • Communicate with Spanish-speaking patients seen in the Carilion Clinic ED (in person or by phone) to follow up after patient discharge, help establish care at Bradley Free Clinic or other more appropriate primary care site, and support patients throughout the process of scheduling and attending appointments.
  • Assist patients with access to other social supports and community resources to overcome social determinants of health such as food insecurity, housing insecurity, healthcare coverage, access to medications, etc.
  • Maintain a database of metrics associated with patient communication.
  • Meet with grant facilitators weekly either in-person or by phone to ensure metrics of the grant are on target.
  • Work within Bradley Free Clinic, ED, and other community sites to meet patients at points of access and connect patients to all levels of care necessary.

Community Outreach

  • Attend community events, reach out to community sites (ie churches, businesses, organizations, etc), and meet with community leaders to foster relationships with the broader Spanish speaking population in the Roanoke Valley and surrounding areas.
  • Aggregate local resources inclusive of legal, food, transportation and government assistance programs that can be provided to Spanish-speaking patients dependent on their needs. This information will be shared with Bradley Free Clinic, Carilion Emergency Department and other partners.
  • Maintain a database of metrics associated with community visits, partnerships and communication.
  • Create promotional materials and messages to encourage patients to access healthcare.

Other Duties as assigned

  • Interpreting and translating documents in Spanish
  • Screening and triaging

REQUIREMENTS

  • The initial pilot period of the position will extend from June 1 - Aug 27, with one week of vacation, for the purpose of collecting data for future funding applications. However, the position will continue after the initial period. Candidates with short-term and long-term availability are encouraged to apply.
  • Candidate must live and work in Roanoke, VA throughout duration of employment
  • Position is 40 hours per week and may require non-traditional work hours
  • This is NOT a remote position
  • Candidate will be provided with a computer for their use during time of employment
  • Candidate must possess a legal, valid driver’s license and have transportation to visit community partners
