JOB PURPOSE

 

The Geriatric Social Worker provides case assistance, emotional support, and group services to older adults residing in senior housing as well as older adult members of the senior center.

 

SCOPE OF INFLUENCE

 

  • Direct contact with client group of older adults within the Sirovich Program and Weinberg Housing;
  • High level of service with internal staff, volunteers, clients, and external partners

                                                                                           

KEY RESPONSIBILITIES AND ACCOUNTABILITIES

 

Provide Case Assistance to Members of Sirovich Center – 65%

  • Assess clients for social work services, and provide case assistance to clients.
  • Assist with obtaining concrete services, and provide crisis intervention when needed.
  • Provide information and referrals to clients and collaborate with community providers.
  • Maintain appropriate case files and documentation for submission to the Department for the Aging (DFTA).
  • Input case notes within 3 days of service provided into PeerPlace, DFTA’s online database.

 

Provide Case Assistance to Residents of Weinberg Housing – 15%

  • Assess residents of Weinberg Housing for social work services, and provide case assistance and referrals to residents.
  • Assist with obtaining concrete services, and provide crisis intervention when needed.
  • Conduct home visits as needed.
  • Maintain appropriate case files and documentation for HUD.

 

Oversee Senior Companion Program – 10%

  • Assess clients for Senior Companion Program.
  • Oversee senior companion volunteers’ time sheets.
  • Match volunteers with clients in need of companions.

 


Other – 10%

 

  • Develop and facilitate support groups in response to client needs.
  • Assist Program Director and colleagues in the day to day operations of group and volunteer services.
  • Attend staff meetings and trainings as required.
  • Work as a member of the interdisciplinary team.
  • Assist in all major Senior Center events.
  • Work one late evening a week.
  • Perform other duties as assigned.

 

 

JOB REQUIREMENTS


 

Minimum Qualifications

 

  • MSW preferred
  • Fluency in either Cantonese or Mandarin 
  • Minimum 2 years of experience working with elderly population, especially resistant and age related cognitive and mentally impaired clients.
  • Knowledge of resources and entitlements, as well as knowledge of age-related cognitive and mental impairments as required
  • Excellent assessment and crisis intervention skills
  • Ability to work in high pressure environment autonomously
  • Demonstrated ability to handle highly confidential information
