What You Will Do:

Assess the nutritional status of patients and manage the nutritional care process, including evaluation and monitoring. To educate patients, physicians and other health professionals. Is cognizant of the philosophy, standards, objectives and policies of the Department and the Hospital Center.

Role & Responsibilities:

  • Demonstrates the ability to perform clinical/technical/service/administrative tasks:
  • Screens, assess and documents nutritional status of patients; assigns appropriate level of care.
  • Plans, develops, implements and evaluates the process of nutrition care plans.
  • Plans, implements and evaluates nutrition education for patient and significant other(s); Develops and revises nutrition education tools.
  • Documents nutrition care provided in patients’ medical record in accordance with departmental standards.
  • Assists in nutritional analysis of menus and modified diets and revision of diet manual.
  • Conducts meal time visits and food intake analysis as deemed necessary.
  • Participates in interdisciplinary patient care rounds and activities.
  • Plans, develops and participates in special projects and committees.
  • Acts as consultant to physicians, nurses social workers, medical students and other disciplines. Presents classes and lectures on nutrition related topics to health care team.
  • After patient interviews, records pertinent eating habits on Kardex file and give to Menu Supervisor to use in preparing patient menu if necessary.
  • Prepares any necessary menu/s and submits to Diet Clerks in Central Diet Office when menu Supervisor is not on duty.
  • Demonstrates the knowledge and skills necessary to provide care, based on physical, psycho social, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area.
  • As part of the grant-funded Food and Nutrition (FNS) program:
  • Conducts outreach to patients regarding enrolling in the FNS program.
  • Completes intake and reassessment visits utilizing NYCDOMH required tools and forms.
  • Creates individual nutrition care plans for patients and reviews and revises treatment plans as needed.
  • Along with the FNS Program Manager, coordinates and conducts FNS nutrition groups for enrolled patients on site and off site.
  • Along with the FNS Program Team, completes outreach calls to patients to keep patients engaged in care.
  • Participates in FNS CQI initiatives.


  • B.S. Degree in Food and Nutrition. Completion of approved American Dietetic Association program for qualifying experience, AP-4 Program or Internship Program.
  • Two years clinical teaching -- three years hospital clinical experience. Maintain Registered Dietitian status through continuing education programs.
  • Member of American Dietetic Association (RD).
