JOB DESCRIPTION:

The Aftercare Case Manager is responsible for the evaluation; needs assessment and follow-up of clients after they have moved into permanent housing; prioritizing these needs and offering specific services up to two years after clients have moved into permanent housing. The position will require occasional evening and weekend work hours and travel throughout the NYC area.

SUPERVISION:  DIRECTOR OF PROGRAMS

RESPONSIBILITIES:

  • Meet with clients one month prior to their discharge to permanent housing to conduct interdisciplinary case conference; pre-discharge exit assessment/engagement plan covering client’s needs and identifying the goals and objectives to maintain independent living.
  • Attends lease signings with clients and schedules first aftercare contact.
  • Conducts home visits to assess clients’ needs and develops a service plan with clients.
  • Utilize Critical Time Intervention with client transitioning to independent living
  • Instructs clients on how to negotiate with landlords, public assistance and other entities as necessary to maintain permanent housing.
  • Assists with the development and maintenance of community resources available to clients in their neighborhood.
  • Maintains appropriate documentation of aftercare services; maintain thorough and accurate records, files and correspondence in written and electronic form.
  • ·        Keeps accurate, up-to-date documentation on all aftercare clients.
  • ·        Assists with any and all data collection systems.
  • Maintains regular contact with clients through home visits and phone contacts.
  • Assists families in crisis, mediates conflicts and identifies appropriate resources within the community.
  • Organize and facilitate quarterly workshops/social gatherings with former residents.
  • Organize and facilitate group mentorship program for interested former residents.
  • Maintain ongoing communications with former clients in order to inform them of educational, training and other opportunities
  • Participates in regular case conferences.
  • Interfaces with other staff to assure communication, consistency and teamwork.
  • Attends all training, meetings and supervision scheduled by supervisor.
  • Performs other duties as assigned by the supervisor.

Sanctuary Model

  • Effectively use and practice Sanctuary Models tools; safety plans, community meetings, psycho-educational groups, team meetings, red flags reviews, and self-care in the program to create a non-violent and trauma sensitive culture.
  • Promote and model the core values of Providence House Mission (Compassion, Non-Violence, Hospitality, and Community) and the Seven Sanctuary Model Commitments to create a non-violent and trauma sensitive culture.
  • Integrate the Sanctuary Model SELF (Safety, Emotional Management, Loss and Future) framework into daily practice to ensure quality care and an effective work environment.

REQUIREMENTS

  • Bachelors of Science in Social Work or related degree.
  • Minimum of 3 years working with homeless families and individuals with multiple needs.
  • Excellent verbal and written communication and interpersonal skills and the ability to work independently.
  • Competent technology management skills, including proficiency with using Microsoft Office; client database management.
  • Required travel within the 5 boroughs

WORK SCHEDULE

  • The Aftercare Case Manager position is a full time, salaried position. Hours worked will be weekdays flexible to accommodate client availability. Evening and/or weekend work may be required on occasion.
  • The office location for this position is Brooklyn, New York.
