Description

Erie Family Health Centers is looking for a committed leader like you to join our Leadership Team! The Director of Patient Programs and Support Services is responsible for leading the development and implementation services to address the social determinants of health impacting Erie patients and their communities.

Responsibilities

  • Oversee and evaluate Erie's patient and community programs, including Maternal- Child Case Management & WIC, Health Promotion, HIV/Hep C services and Food Insecurity programming
  • Identifies and pursues strategic partnerships to support these initiatives
  • Responsible for community outreach including coordinating Erie�s participation in health fairs and external educational events
  • Plan and implement new projects and programs in accordance with health center goals and service delivery initiatives
  • Accountable for budget development, financial performance, productivity, people management practices, quality management, quality improvement, regulatory compliance and achievement of the goals and objectives of the programs
  • Supervises and coaches staff; communicates and supports organizational goals and objectives within the programs and participates as a member of the health center's general management

Requirements

  • Completion of a bachelor's degree in health care, social work, public health or business administration, required. Master's degree (MPH, MSW or MHA) preferred
  • Five to Seven years of progressive management experience, in a health care delivery organization or social service setting
  • Ability to communicate effectively in both English and Spanish, preferred (May be required to assist or communicate with non-English/Spanish only speaking patients and/or guests)
  • Ability to plan, organize, implement, evaluate, and modify program needs
  • Ability to communicate effectively in speech, and writing, with individuals and groups
  • Capacity to successfully manage quality improvement activities
  • Motivate, build teams and work collaboratively with individuals and groups consistent with the mission, vision, and values of Erie Family Health Centers
  • Ability to problem solve and address issue as they arise
  • Ability to work with staff members across all levels of the agency
  • The cultural competency to work effectively in multi-cultural, low income, minority environment
  • Passion for working with an underserved population and developing and supporting a management team toward a set of shared goals
  • Enjoy working as part of a collaborative team to achieve high levels of productivity and maximize available services
