Under the supervision of the Executive Director, the Intensive Care Coordinator (ICC) Case Manager is responsible for coordinating and providing Intensive Care Coordination and recovery support services to individuals at high-risk for an overdose across all BHLI Project Connection sites. The Intensive Care Case Manager will work as a member of the Management Team to implement BHLI’s mission to improve public health to the most vulnerable populations, to increase accessible treatment, and to reduce overdose deaths of people with Opioid Use Disorder by reducing gaps in the continuum of accessible high-quality treatment. 

Essential Responsibilities

Essential and other important responsibilities and duties may include, but are not limited to the following:

  • Collaborate with the BHLI Management Team to achieve goals including the provision of high quality, effective and efficient services are provided to patient population.

·        Coordinate and provide Intensive Care Coordination and recovery support services to individuals at high-risk for an overdose, to include assistance with “warm hand-offs” for individuals needing access to Medication Assisted Treatment to another community provider.

·        Ensure eligibility criteria among BHLI clients for ICC services. Including reviewing DSM5 criteria of Opioid Use Disorder – Moderate/Severe. 

·        Work with consumers enrolled in BHLI’s low-threshold and PCARE van programs.

·        Assist with connecting consumers with services and resources within the community.

·        Help consumers prepare for and complete the transition to continuing care physicians for ongoing buprenorphine maintenance treatment and ongoing medical care.

·        Be knowledgeable of resources available to Baltimore City residents that can assist with linkage to treatment services.

·        Engage in monthly and quarterly reporting to funders and larger management team. 

Minimum Qualifications

  • Master level professional with 3 + years related Case Management with experience in >2 community-based settings preferred. Experience may be considered in lieu of education.

·        Have knowledge and/or experience with Medication-Based Treatments for Opioid Use

·        Demonstrated understanding of harm reduction values preferred.

Knowledge, Skills and Abilities

  • Excellent oral and written communication skills.
  • Ability to articulate BHLI’s mission through his/her work.
  • Experience with underserved population or other special needs populations.
  • Experience with working in Substance Use Treatment
  • Demonstrated experience in using good judgment in working with diverse populations.
  • Demonstrated ability to prioritize activities.
  • Understanding of the importance of data to decision-making and facility with data and research approaches.

·        Ability to creatively approach problems, use out of the box thinking with solution-oriented behavior and help staff to utilize all available resources to overcome barriers, address social determinants of health to support positive patient outcomes.
