Adult Home Referrel Program                 


Communilife is seeking a qualified professional to join our team of professionals at our Queens, NY location.


Reporting to the   Assistant Vice President – Programs, the Program Director is responsible for the overall program operation of Adult Home Referral program which includes management & supervision of staff, overseeing the development and implementation of program enhancements, monitoring and compliance, evaluation of policies and implementing QI and incident report procedures for the program. The Program is characterized by a team approach, supervision of clinical staff, time unlimited services, crisis availability, and referral procedures. The Program Director is on call 24/7. 

ESSENTIAL FUNCTIONS AND KEY ACCOUNTABILITIES

  1. Plans and administer daily operation of the program
  2. Responsible for staff program assignments and supervision of program activities to ensure that clients receive services in accordance with their needs
  3. Assist and participate in the recruitment and retention of staff
  4. Responsible for enforcing policies and procedures in compliance with Federal, State and City Laws, rules and regulations
  5. Ensure that Quality Improvement Policies and Procedures are follow as required

6.  Review and accept new and renewal leases or assignment of leases from other contractors

7. Provide oversight to ensure that a minimum program monthly occupancy rate of 95% is               maintained

  1. Ensure program compliance with OMH contract requirements
  2. Regularly review and monitor assessors output activities to ensure that benchmark are met.  
  3. Regularly review Inreach work performance in meeting the monthly contractual outcome.
  4. Monitor Inreach list, generated in CAIRS to ensure that all efforts to engage class members have been properly documented.  Provide and maintain records of staff trainings on an annual basis to increase their knowledge and understanding of OMH Adult Home Initiative, older population, psychotropic medication and clinical intervention with the older adult population, mental illness and substance use intervention, and harm reduction techniques. 
  5. Provide oversight in maintaining and securing individual files and records to ensure that files are kept complete, current, and readily identifiable from other Comunilife programs. Ensure that all client records are maintained by Comunilife for a minimum of seven (7) years after the expiration of the contract.
  6. Maintain all documents necessary for a quality assurance and improvement review by Comunilife or OMH.
  7. Provide reports as requested by Comunilife and/or OMH. Ensure timely completion of post incidence, benchmark and weekly occupancy reports. Provide oversight in conducting an annual Client Satisfaction Survey and provide surveys to HRA if requested
  8. Ensure policies, principles, and procedures are in agreement with OMH requirements, guidelines, regulations and directives
  9. Lead team organizational and service planning meetings, provide direction to staff regarding individual cases, conduct side-by-side contacts with staff, and regularly conduct individual supervision meetings
  10. Meet on a regular basis with the OMH staff for the weekly supervisory conference call and planning purposes for better cooperative working agreements. Maintain contact with various community program administrators, public officials to discuss and at times negotiate cooperative working agreements
  11. Provide oversight for consumer goals of recovery, rehabilitation, economic independence, stability and community integration.
  12. Oversee the coordination of comprehensive health services with other community services such as inpatient, outpatient, emergency, and LMTC, health home, PROS, and design services to close gaps in the continuum of services
  13. Design and implement a comprehensive program evaluation. Evaluate/review employees’ annual job performance.
  14. Prepare any and all correspondence relative to the Adult Home Referral program as requested by the State, City or other funding sources. Responsible for addressing and correcting any and all deficiencies, which may be cited as a result of audits conducted by the funding sources. 
  15. Assist the Assistant Vice President of Programs or designee with special projects, analyze, research, and prepare reports as required.
  16. Perform other duties as assigned

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES

ü Extensive knowledge of principles and practices of Adult Homes, OMH Adult Home Initiative, older population and mental illness.

ü Ability to provide immediate clinical intervention (as needed) to clients affected with mental health, chronic medical, and substance abuse.

ü Thorough knowledge of program development, implementation methodology, and research methods and techniques.

ü Ability to plan, organize and direct the work of subordinate clinical and administrative staff. Responsible, organized, and efficient. 

ü Ability to perform in a team approach and accomplish multiple assignments simultaneously.

ü Communication skills (effective/active listening, verbal and written)

ü Community minded, patient, creative, flexible, and compassionate. Culturally sensitive to persons who are diagnosed with severe mental illness, various national/ethnic backgrounds, sexual orientation, religion, and substance abuse issues.

ü Ability to deal tactfully and diplomatically with other Comunilife employees, Federal, State, and City officials, professional and technical groups, clients, and the general public.

JOB SPECIFICATIONS

Education and Experience Requirement(s)

A Masters Degree in social work, public health, health administration, nursing or  related field; and three(3) to five (5) years of administrative and supervisory experience operating Scatter Site Housing Program(s); or Bachelors Degree in  social work, public health, health administration, or  related field; and five(5) to seven (7) years of administrative and supervisory experience operating Scatter Site Housing Program(s).  Five or more years experience in providing direct services related to the treatment and recovery of older adult population with serious chronic illness including medical, mental health and substance use. Bilingual-Spanish preferred. Gerontology training a plus.

Working Conditions

Field work as required for community relations. General office environment.

Certifications & Licenses Required  

LMSW/LCSW, RN, and/or CASAC.

 This is a full-time minimum thirty-five (35) hours per week position. Core office hours are 9:00 a.m. to 5:00 p.m. Monday to Friday. Flexibility (evenings/weekends) is required to meet the program’s operational needs.
