Aging and Disability Supervisor
Closing Date: March 5th, 2021, or until filled
Neighborhood House has a longstanding commitment to ensure that low income, immigrant and refugee aging and disabled members of our community have the opportunity to age in community, receiving culturally appropriate services that enable the highest quality of life. The AD Supervisor has the primary responsibility for successful implementation and oversight of our Long Term Care Case Management program, which provides clinical support to older adults, individuals with disabilities, and their caregivers. The AD Supervisor would provide direct supervision of the case management staff working with clients from low-income, immigrant and refugee communities. The AD Supervisor provides oversight of all day-to- day processes for the program including coordinating case transfers, supporting case management duties, ensuring monthly program goals are met, and overseeing program performance. The AD Supervisor will be the primary contact for all program audit activities, and will work closely with the Program Manager on program improvement initiatives. The AD Supervisor will lead the case management team, providing policy-informed, solution-oriented support to case managers toward the shared goal of delivering culturally and linguistically competent services to the program’s clients and their caregivers.

- Oversee the supervision of the direct service team, ensuring all program regulations, protocols and client services are in compliance with Neighborhood House and funder guidelines.
- Provide solution-focused oversight and coaching to direct service staff to help meet program goals, problem solve, and ensure effective delivery of services to clients and their families.
- Work closely with Manager and program leadership to develop and monitor documentation systems for tracking the multiple deadlines associated with the administrative requirements of the program. Train other staff on proper use of these systems; monitor for compliance.
- Develop, monitor, and enhance internal and external referral systems to ensure coordinated referrals into the program and among direct service staff to ensure equitable caseloads and manageable expectations for staff.
- Analyze and evaluate program quality and client services, making modifications and adjustments to ensure the best possible client services.
- Supervise team members, delegating and distributing work to maximize staff potential and program budget.
- With support from the Manager, regularly observe staff, appraise ongoing performance, and provide regular, motivational formal and informal feedback.
- Ensure all required staff certifications and training requirements are up to date.
- Monitor and review new program requirements adjust policies accordingly and report to Manager regarding updated policies.
- Coordinate staff trainings based on observations and analysis of staff needs, , Manager, Program Director and funder requests.
- Maintain frequent communication with Manager to discuss challenges in process or barriers to meeting program goals.
- Be available to agency staff to answer questions and troubleshoot problems.
- Maintain, review for accuracy, and approve key project documents, including regular program reports, evaluations, and satisfaction surveys.
- Assist and plan for alignment of ADS programs with the agency’s No Wrong Door philosophy and practice.
- Participate on NH leadership teams and/or workgroups as necessary.
- Represent NH by participating in activities or professional networks related to aging and disability services or other related networks as assigned.
- Perform other duties as assigned.
- Bachelor's Degree in social work, social sciences, health, human services or related field. An equivalent qualification may be considered in combination with the required experience.
- Three years of experience working in social services or health field with an emphasis on aging, low- income and diverse populations.
- More than two years of experience supervising a large, diverse team in a complex program.
- Demonstrated ability to lead a team through understanding and adhering to complex Federal and State regulations and definitions and to complete extensive technical documentation.
- Very organized with a high level of attention to detail.
- Strong professional and ethical judgement, particularly in the human services context.
- Ability to collaborate with leadership to create efficiencies and maximize staff potential.
- Ability to communicate effectively, both orally and in writing, with individuals from a wide range of cultural backgrounds.
- Self-starter, highly organized, dependable, have strong problem-solving skills and be able to manage multiple complex problems.
- Must be proficient in key computer applications (MS Office/Word/Excel/Access).
- Must have independent reliable transportation.
- Current First Aid Certificate (within 30 days of hire, maintain current certificate during employment).
- Current CPR Certificate (within 30 days of hire, maintain current certificate during employment).
- Sexual Harassment Training within 30 days of hire.
- Child Abuse and Neglect training (within 2 weeks of hire).
- Must pass Neighborhood House’s background check requirements.
- Position requires Washington State bi-annual criminal background re-check.
- Master’s degree in social work, social sciences, health, human services or related field.
- Prior work experience in state-funded health programs such as Medicaid, Medicare, Health Home, CHIP, etc.
- Knowledge of and proficiency in HIPAA-compliant, client services-tracking database or related database navigation.
- Knowledge of and experience in older adult or disability issues and social or health related resources that assist older adults and individuals with disabilities maintain their independence and quality of life.
