Description

Background : Join our dynamic team based in the historic Mayfair neighborhood of East San Jose. Make a difference in the lives of Mayfair residents who often work in low wage jobs, where families live in a single room or garage, and are unable to access government aid due to their immigration status.


Position Summary: The Case Manager will conduct comprehensive client assessments, evaluate their needs and create short and long term housing goals with the ultimate goal of finding permanent housing. The candidate will also screen, place, and case manage individuals for safe park. The position requires a high level of independent decision making, action, and responsibility. The overall goal of the Case Manager is to enhance the quality of the client's life by moving them out of poverty as defined by the self-sufficiency matrix.


Essential Functions:

-Assess and verify client needs, determining eligibility for a variety of financial and in-kind emergency services

- Provide case management services to all low-income clients who qualify, including goal setting, long-term case

plan development, process monitoring, money management, tenant education and referrals

-Maintain a caseload of 50 families per year(average case load is 20-25 at any given time)

-Meet with clients weekly or bi-weekly for 12 months and track their progress

-Connect families with needed and available community resources. Follow-up with clients and agencies as

appropriate to document use/success of referral.

-Coordinate with various service providers to ensure the fulfillment of what the clients need

-Establish and input accurate and complete data for all contacts with clients into agency database

-Provide all required documentation in a timely manner, which may include client follow-up, outcome evaluation,

client contact sheets and evaluations.

-Plan and coordinate health related educational events with health providers

-May supervise interns for special projects


Other Functions:

-Participate in regular staff meetings, staff training programs, and supervisory meetings

-Adhere to agency policies and procedures

-Other duties as assigned


Minimum Qualifications:

-BA or Masters in Social Work, preferred

-Two years experience working in a nonprofit setting

-Experience with case management of families

-Ability to work in a variety of settings with culturally diverse individuals and families and be culturally sensative

-Knowledge of community resources and counseling

-Excellent documentation skills

-Ability to lift 25 lbs.
