Heights and Hills, a community based agency providing services to Brooklyn's older adults and their families for over 40 years, has an opening for a qualified, committed LMSW or LCSW to be responsible for our Caregiver Support program. Our mission, to promote successful aging in the community, is guided by the beliefs that older adults are assets to their communities; are entitled to live as independently as possible, with dignity and choice; and that professionally delivered home and community-based services for older people are essential. We serve homebound older adults and their families in many Brooklyn neighborhoods, providing them with the information, support and services they need to live their lives on their own terms.

Caregiver Program Director

Job Type: Full-Time

Role

The Caregiver Program Director is responsible for overall administration and supervision of the day-to-day operations of the Caregiver Program, and provides administrative and clinical supervision to staff and student interns. In addition, the Caregiver Program Director will represent the agency in community and interagency activities. The Caregiver Program Director works under the supervision of the Director of Program.

Responsibilities

  • Oversee screening and intake of all new potential caregiver clients as well as providing this function directly.
  • Assign caseloads and related duties
  • Provide clinical supervision to caregiver staff, including four licensed social workers and one part-time outreach worker
  • Develop and cultivate community and corporate contacts
  • Provide public presentations and training sessions on caregiving topics
  • Oversee and monitor the provision of respite and supplemental services
  • Review case records for clinical appropriateness as well as compliance to standards as established by the Department for the Aging
  • Lead regular group supervision for caregiver staff
  • Oversee and manage the program’s budget
  • Track and monitor program deliverables
  • Compile statistics, prepare reports and other required records
  • Work with the Director of Programs on special projects
  • Work with Director of Programs to coordinate in-service training program for staff
  • Work with the Executive Director and Executive Management team on outreach, program planning and development
  • Represent agency in community activities
  • Perform other duties as required

Required

  • LMSW or LCSW with a minimum five years of experience
  • Minimum two years’ experience supervisory experience
  • Have an awareness of the biological, psychological, and social aspects of aging and the impact of disabilities and illnesses on aging and the family
  • Excellent organizational skills
  • Must possess excellent clinical skills
  • Good presentation skills
  • Ability to arrange and negotiate service referrals
  • Skills in establishing and sustaining interpersonal relationships
  • Proven ability to work both independently and as part of a team
  • Must be able to work occasional evenings and weekend days
  • SIFI certified or eligible to obtain SIFI certification in first year
  • Excellent computer literacy – Windows, Microsoft Office
  • Must be able to travel on public transportation to do community presentations and home visits
  • Must be able to climb steps to do home visits

 Preferred

  • Knowledge of Department for the Aging services and programs
  • Knowledge of Department for the Aging database (STARS)
  • Prefer bi-lingual and cultural competency in Spanish, Chinese or Haitian Creole. 
  • Knowledge of community resources and public benefits eligibility
  • Hospital or community experience working with the elderly and/or their families
