Case Managers at Housing Works provide comprehensive support to clients with the goals of minimizing barriers to care, improving client health outcomes, and helping clients develop the ability to manage their needs with greater independence. Working in both the community and in office settings, Case Managers work collaboratively with an integrated team of professionals to provide client centered, outcome driven care.
This is an exciting opportunity for Bachelor’s or Master’s level human service professionals who want to have a direct impact on the lives of people in need while also gaining valuable experience in working with issues of chronic illness, homelessness, HIV, mental health and substance use.
