The Bridge, an innovative and well respected mental health agency serving adults with serious mental illness, substance abuse, co-occurring mental health and substance abuse disorders, and co-morbid medical conditions is seeking a Clinical Coordinator for its ACT Program.


  • Obtain Pre and Concurrent authorization and conduct Utilization Management in communication with Managed Care Organizations; evaluate medical records for accurate and complete documentation of medical necessity, treatment modalities and appropriate level of care;
  • In collaboration with Team Leader and QA Clinician Conduct utilization review and quality assurance reviews to ensure that the ACT team operates in compliance with New York State Office of Mental Health guidelines and regulations and New York City Department of Health and Mental Hygiene operating standards;
  • Update Medicaid and Managed Care information in the clients record in AWARDS and other relevant databases;
  • Tracking eligibility and entitlements for clients in conjunction with designated team members;
  • Utilize the AWARDS EHR to review and analyze clinical data; build and amend forms that enhance collection of appropriate clinical and demographic consumer data; build and utilize reports;
  • Tracking eligibility and entitlements for clients in conjunction with designated team members;
  • Work collaboratively with Compliance and Billing departments
  • Conduct HARP and HCBS Eligibility Community Mental Health Assessments with ACT participants in their homes, in the program office, or anywhere in the community identified by the participant;
  • Participate in State sponsored trainings and meetings to stay current in usage of the community mental health assessment;
  • Collaborate with ACT Directors and ACT staff to ensure findings from community mental health HCBS eligibility assessment are integrated into ACT participant transition plans;
  • Assist Team Leader and QA clinician in collection and reporting of biannual OMH Patient Characteristic Survey data;
  • Coordinate reporting with Health Homes;
  • Exercise appropriate judgment in answering questions and releasing information; analyze and project consequences of decisions and/or recommendations;
  • Participate effectively in team processes;
  • Work directly with clients to apply for and maintain entitlements
  • Perform other duties as required by Directors and Executive Staff.


  • LMSW or Master’s degree in psychology or related field, with professional NYS clinical license; • Must have or be eligible to have NY State ID (non-drivers or drivers license); • Knowledge and experience of AWARDS, CAIRS, HMIS. • Ability to create and manipulate spreadsheets in Excel • Knowledge of ACT and current psychiatric treatment modalities desired • Knowledge of Diagnostic and Statistical Manual classifications • Experience with reviewing, auditing medical records and familiarity with ACT documentation standards, • At least two years of experience providing services to adults with serious mental illness; and • Spanish speaking a plus
