    CASE MANAGER

    SUMMARY:

We are looking for a dependable, compassionate, justice seeker to join our team as a Case Manager, and help support children and families experiencing foster care. Sound like you? Perfect. Keep reading....

Atlanta Angels is an innovative nonprofit that seeks to change the way children, youth and families experience the foster care system. We believe that if we can support every child, youth, and family in the foster care system into our programs, we can truly transform not only the foster system, but the world we live in. Our fast-growing organization has a start-up mentality, but also a level of professionalism and drive that is not found in most nonprofits. There is a LOT of opportunity with our company and we are looking for a creative, innovative self-starter who exudes the core competencies of professionalism, passion, positivity, initiative, and adaptability as well as the core values of hope, community, empowerment, curiosity, and transformation.

    HISTORY:

Established in 2010, Austin Angels wraps community around children in the foster care system, as well as their caretakers, by offering consistent support through intentional giving, relationship-building, and mentorship. Our programs are created to help empower children and build sustainable foster homes. Our solution to the foster care crisis are the Love Box family support program and Dare to Dream youth mentor programs. Our programs utilize a trauma-informed, wrap-around service model that strengthens relationships between foster families, youth in foster care, and bio families. Atlanta Angels launched the Love Box and Dare to Dream programs in metro-Atlanta in 2021.

    GENERAL OVERVIEW:

The main role of a case manager will involve matching selected volunteers and mentors with foster families and youth in foster care, through facilitation of a strength assessment and initial meetup to establish baseline needs of the youth and families that we serve; engaging in ongoing trust-based relationship and management of the match; maintaining documentation; and tracking outcomes.

    PRIMARY RESPONSIBILITIES:

    General Case Management

    • Work as directed to support the mission and values of Atlanta Angels.
    • Engage in discussions around sensitive topics including child abuse, neglect, trauma, mental health, poverty, substance abuse, trust-based relational intervention, rehabilitation, social adjustment, therapeutic needs, child demographics, case trajectory, family preservation and child relational concerns. Utilize these discussions to advocate and make decisions for the best interest of the client.
    • Gather foster family referrals and/or foster youth information including their background information, needs, and child information to determine the best volunteer match to meet their needs.
    • Work with the volunteer coordinator to identify the best match for family/youth.
    • Facilitate strengths assessment/initial meetup between foster family/youth and volunteer/mentor and conduct training for volunteer groups/mentors. These may be held in the evenings or on weekends due to the volunteer/foster family availability.
    • Maintain minimum monthly check-ins with families, youth and volunteers. Advocate for access to resources to meet varying needs. Provide support and conflict resolution as necessary throughout the length of the relationship.
    • Build and maintain partnerships with local CPAs (child placement agencies) and other child welfare organizations.
    • Serve as liaison between volunteer, foster family and placement agency.
    • Identify and collaborate with other local partners to provide resources to families and youth referred to Atlanta Angels.

    Documentation and Admin

    • Maintain case history records and prepare impact reports from testimonies of foster families, youth, and children served through programs.
    • Maintain accurate documentation and case notes in Apricot.
    • Conduct ongoing monitoring of volunteer contact and tracking of relational outcomes for children/youth in the Atlanta Angels program.
    • Resolve program complaints and requests to change volunteer matching arrangements.
    • Create and send monthly tracking forms and newsletters with updates and provide program direction.

    Program Events and Special Initiatives

    • Assist in planning program events as it relates to communication and coordination with families/youth.
    • Maintain record keeping of impact for program initiatives.
    • Attend and serve at events including fundraising, community awareness initiatives, events for families, youth, and children which often occur over weekends or evenings. These events include Sunshine Festival, Back-to-School, Holiday, and gala.

    KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:

    • A qualified candidate will be a highly skilled communicator with strong interpersonal skills and abilities to quickly curate trust based connections (including foster families, foster youth, volunteers, mentors, corporate partners, and other Atlanta Angels supporters). This person will possess knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; and psychological needs.
    • Case managers should demonstrate a spirit of inclusivity and acceptance of people from all backgrounds and walks of life.
    • A qualified candidate will be a self-starter who can work independently and manage multiple tasks simultaneously.
    • A qualified candidate will have the ability to be teachable in the different skills the job requires.

    WORK CONTEXT:
