Background

We are actively recruiting a Social Care Coordinator to help manage and build UniteNYC, a network of health, human, and social service providers in New York City, powered by the Unite Us technology Platform.

This exciting opportunity is a new role within AIRnyc, and we are looking for a pro-active self-starter who will help identify in the Unite NYC network to serve our patients and other NYC social and health  care providers. The Coordinator will be (or become) a Unite Us Platform expert and connect with community-based organizations to ensure successful closed-loop referrals. The Coordinator will also identify gaps in services or disconnects with the referral process and work with Unite Us staff on identified issues. The Social Care Coordinator will also assist in the administrative functions necessary to set up meetings, document action steps and develop plans to implement priorities to achieve the overall goal of improving the platform.


Key Characteristics of a Successful Candidate

·      You are a quick learner with knowledge of the social service landscape in NYC

·      You have experience working with underserved communities

·      You have excellent communication and interpersonal skills and will be able to work with a diverse group of stakeholders including Community Health Workers, Community Based Organizations, the AIRnyc leadership team and technologists from Unite Us and other partner organizations

·      You have strong organizational skills in the context of setting priorities, carrying out a project plan, and escalating issues to supervisors

·      You are a team player and problem solver with a sense of humor

·      You have a desire to learn

 

Duties and Responsibilities

Under the supervision of a manager, the Social Care Coordinator will be responsible for the following:

·      Tracking needed referrals that are out of the network and identifying they types of organizations that should be added to the Network to better serve the entire UniteNYC community.

·      Support AIRnyc Community Health Workers and their use of the Unite NYC platform.

·      Attend regularly scheduled group meetings with other CHWs, program supervisors, Unite US and the UniteNYC network members.

·      Interact with other social care service providers on individual cases as needed

·      Meet regularly with a supervisor to track progress, identify priorities and provide feedback to relevant stakeholders.


Requirements

·      Strong verbal and written communication skills (IRL, Zoom and on social media)

·      Ability to track progress, prioritize and execute on tasks

·      Experience using database technology, including the ability to organize, enter, analyze and report on data collected


Desirable Skills

·      Knowledge or prior experience utilizing social care networks such as Now-Pow, Aunt Bertha, or Unite Us

·      Experience with Community Based Organizations, public health agencies, and/or health care delivery organizations

·      Salesforce experience a plus
