The case manager has to have a background in both educational and practical experience in working with individuals that have a history with the criminal justice system. In addition, this staff person must be able to address the specific needs of individuals with a history of alcohol or other drug abuse/addiction and make appropriate referrals as required. Preference will be given to an individual with at least two years experience in the human service field and must have a valid NYS driver’s license. Training will be provided by the Program Administrator. This position requires either a CASAC, MSW or LMSW.

The case manager operates under the direct supervision of the Program Administrator and assists in the daily operation of house and program.

The case manager must have working knowledge of community resources, especially in Nassau County, housing services, employment/vocational programs, treatment/medical agencies, self-help groups, etc. in order to make the most appropriate referral that will meet the specific needs of the participant.

The case manager must possess writing/computer and communication skills in order to develop a transitional plan and prepare required documentation. This staff person must have the ability to teach/educate the client by effective communication and guidance on how to access and utilize community resources that will foster positive growth and change in the participant’s life.

Case manager must be flexible in order to accomplish an array of diverse duties that in addition to the aforementioned will include driving/accompanying participants to appointments as applicable.

This is a full-time 30 - 35 hour position with benefits at a MEN'S Transitional House that requires evening hours and weekends and to live in Nassau County. Dismas House is operated by the Society of St. Vincent de Paul whose mission remains central to growing spiritually through service to the poor.
