Jubilee operates ten properties totaling 299 units of affordable housing and offers robust programming for residents and community members pursuing life goals.

POSITION DESCRIPTION:

 

In 2011 Jubilee Housing opened the Re-entry Housing Initiative which is a transitional housing program for criminally involved (returning citizens) DC residents. We currently operate two homes in the Adams Morgan and Columbia Heights neighborhood, one for men and one for women. Outreach is a very important aspect of how Jubilee ensures that our residents are receiving the maximum support and benefit of our program and the external community.

Community outreach and having strong community partners has always been an integral part of our service delivery at Jubilee.

 

The Community Outreach Liaison is a critical role in the multidisciplinary approach of Jubilee Housing’s Re-entry Housing Initiative (RHI). This position is responsible for initial outreach to potential program participants, informing them about the benefits of the program in their plans for successful re-entry outside of incarceration including support for addiction recovery, employment, and constructive decision-making.  This staff person will spend 50% of time traveling from Washington, DC to state and federal prisons in West Virginia, North Carolina, and Philadelphia to conduct program presentations to DC men and women, small groups, and individuals to build interest in the program and to inform candidates about the necessary steps to enter and complete the program before and following their release. This staff position is expected to produce at least 10 viable program candidates monthly, who will enter the Re-entry program fully informed and ready to participate.  The Community Outreach Liaison will also garner resources for the development and execution of the Re-entry program including food, clothes, educational and job training programs and long-term housing options.

 

REPORTING & WORKING RELATIONSHIPS:


This position is supervised by the Program Director and works collaboratively as a member of the Re-entry Housing Initiative team.


ESSENTIAL JOB FUNCTIONS:


Ø Knowledge of the unique issues affecting high-risk communities and willingness to learn about culturally relevant interventions for individuals who have abused substances or have other addictions, mental health diagnosis’s, possible health disparities,  or HIV/AIDS.

Ø It is critical that the Community Outreach Liaison build trust with community organizations and other re-entry stakeholder organizations to solidify the referral processes for program resources and development. This function of the position will assist program participants in sustaining services that will assist with their self-sufficiency once they exit our program.


External Functions:


Ø Ability to travel weekly into the DC Department of Corrections Facilities.

Ø Ability to travel into the Federal Bureau of Prison facilities on a quarterly basis for up to three nights.

Ø Participate in the recruitment and retention of program participants which will include individuals who have co-occurring disorders.

Ø Represent the organization at community outreach events and other opportunities to build awareness or support for the program.

Ø Provide linkages and referrals to permanent housing organizations, apartment complexes and any other social services that benefit program participants.

Ø Help foster community involvement, support and effective collaboration for the program.




Internal Functions:


Ø Conduct an assessment for all individuals who inquire about the program.

Ø Collaborate with the Case Manager and Certified Addictions Counselor to determine individual service needs prior to entrance.

Ø Make referrals to appropriate health and human services agencies as needed based on the outcome of the screenings, interviews, and advisement of program staff.

Ø Work with Director to create and implement an effective outreach plan.

Ø Connect residents and the community to our programs and other longer-term housing options.

Ø Introduce potential residents and current residents to program services that can aid in their long-term success while participating in the program and after exiting.

Ø Identify and coordinate community outreach activities.

Ø To determine viable program candidates, conduct confidential, quality assessments and interviews, that are gender-specific and trauma informed, using the appropriate level of care in the community, at Jubilee offices or during screenings and interviews within the DC Department of Corrections and the Federal Bureau of Prison facilities.

Ø Collect and review referral information to ensure program staff has all the needed documentation to make informed decisions.

Ø Collect information from resources to aid in determining the appropriateness of the resources.  

Ø Assist with data collection and reporting regarding program services executed and resident information.

Ø Screen and make referrals to the most appropriate programs and agencies in an effort to assist residents in accessing the right level of support to best meet their needs.

Ø Participate in all assigned meetings, staff development opportunities, and trainings as required.

Ø Provide back-up assistance to program team members as needed.

Ø Other duties as assigned

 

DESIRED QUALIFICATIONS:

 

Ø Bachelor’s degree in Social Work, Human Services or Sociology and four years of experience working with returning citizens, preferred with consideration for comparable skills, experience, and education.


Ø Demonstrated knowledge of DC services and processes for acquiring services related to education, mental health, substance abuse in-patient and out-patient treatment services, and vocational training.

Ø Experience working with individuals who are or have been criminally involved (returning citizens).

Ø Understanding and supportive of a trauma-informed system of care.

Ø Strong interpersonal, engagement, organizational and time-management skills with the ability to work independently with little supervision.

Ø Excellent written and verbal communication skills.

Ø Strong computer skills required.

Ø Ability to support the organization’s mission and philosophy, and demonstrate sensitivity to cultural diversity.

Ø Value for and the ability to deliver excellent program services.

Ø Valid driver’s license and clean driving record, required.
