Department: Behavioral Health Services

Reports To: Manager of Behavioral Health Services and Psych APN.

SUMMARY

Participates in the assessment and treatment of the clinical needs of the clients. Assists in the development of appropriate treatment plans and assumes overall psychiatric case management responsibilities for assigned clients.

DUTIES AND RESPONSIBILITIES

  • Engage target population through in reach and outreach activities with the Vaccine Task Force.
  • Conduct tailored screenings and assessments designed to identify vaccine attitudes/hesitancy, mental health, substance use, and any other behavioral health needs.
  • Make referrals to the Psychiatrist when necessary.
  • Provides individual, group and family psychotherapy as clinically needed to assigned clients. Mental health counseling sessions shall include overcoming vaccine hesitancy through trauma informed care and treatment.
  • Involves client and/or family in developing achievable and understandable psychosocial treatment plans in accordance with accepted professional standards.
  • Advocates for appropriate resources for clients and families based on assessed needs.
  • Engages clients in appropriate treatment program.
  • Assists clients in effectively managing crises.
  • Responds with sensitivity to cultural and socio-economic issues of clients. Manages termination issues in an effective and timely manner. Manages caseload demands effectively while considering both quality of care and agency needs.
  • Assumes overall psychiatric case management responsibilities for assigned clients, including coordination of treatment and communication with other treatment service providers.
  • Complete treatment plans, progress notes, case notes, and discharge summaries within the required periods as stipulated by program policy.
  • Document services in EMR (Athena) and other databases provided by the funder.
  • Collaborates effectively with other staff members, referral sources and social support networks to provide treatment and care for clients and families.
  • Maintains effective, open communication with members of the treatment team.
  • Maintains and completes appropriate client records, charts and reports in a timely manner, in compliance with the DMHAS standards and other funders.
  • Completes termination/discharge summary within 15 days of termination of services.
  • Maintains knowledge of trends and developments in the field, by attending meetings, training events and educational workshops and seminars.
  • Participates in agency meetings, committees and task forces and contributes to smooth functioning and operations of the department.
  • Provides direct clinical and administrative supervision to students and interns. Evaluates performance of those supervised to determine areas that need improvement.
  • Keeps abreast of all federal, state and NJCRI regulations, laws and policies as they presently exist and as they change or are modified.
  • Performs other duties as assigned.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE

Master’s Degree in appropriate mental health discipline plus two (2) years post master is experience in the mental health field, or Doctorate. Maintain an active license in clinical social work (LCSW) Experience in counseling with HIV and at risk persons. Solid HIV/AIDS knowledge base. Excellent leadership, interpersonal skills and creative problem solving skills.
