PURPOSE OF POSITION:

The CAN Coordinator will work as the support for activities to plan, facilitate, and document an efficient, effective and collaborative homeless response system throughout Northwest Coordinated Access Network (CAN). The Northwest CAN is a partnership of homeless and housing providers, funders, and other key stakeholders with the shared goal of ending homelessness in the region. This position will work with various members of the homeless response system and community partners to advance the local work. The CAN Coordinator must maintain collaborative and cooperative relationships with Northwest agencies and partner agencies of the CAN and must participate in Balance of State or other statewide meetings, as needed. 

RESPONSIBILITIES:

· Coordinate housing solution meetings using the CAN approved process:

·     Facilitate the Housing Solution Meetings (i.e. managing invites, agendas, running meetings, etc)

·     Engage other housing providers in the process and develop a positive collaborative working relationship to elicit buy-in and commitment

·     Present an up to date By-Name List (BNL) of each population most in need of housing at each meeting to work with community providers in matching clients to appropriate housing opportunities

·     Serve as “gatekeeper,” working to ensure all side doors are closed and ensuring that households being matched to any available housing in the community are prioritized based off the statewide prioritization

· Coordinates CAN functions with staff from participating agencies

· Involvement in youth work to ensure coordination between the CAN and Youth/ Young Adult providers

· Maintains positive, collaborative relationships with Northwest CAN members and leadership, public and private partners, DOH, DMHAS, and Balance of State.

· Establishes protocols to track and analyze CAN performance and outcome data; creates reports and communicates. 

· Maintains accurate record keeping in HMIS.

· Collaborating with local housing specialists in landlord engagement.

· Maintain an up-to-date housing inventory within Waterbury/ Litchfield.

· Keeps abreast and knowledgeable about issues and trends of homeless services and reports back to Waterbury/ Litchfield leadership team.

· Prepares and distributes monthly agenda and any other meeting materials for CAN leadership meetings. 

· Attend, lead, and take minutes at CAN leadership meetings, and any other identified meetings.

· Takes recommendations from Leadership and advocates to DOH and other statewide partners.

· Work to optimize all community resources and services. 

· Regional PIT Coordinator to be the liaison between the CAN and CCEH, organize volunteers, and provide assistance to the sub-community PIT coordinators

Level of Language Proficiency

Bachelor’s Degree in Social Work or other related field with a minimum of 5 years’ experience in working with the homeless or at-risk population including experience in program management. Must have the ability to work on a community level and engage in a collaborative approach. Bilingual preferred.
